彩色多普勒超声检查在诊断椎动脉狭窄中的应用价值

摘　　要：目的:评价彩色多普勒超声检查在诊断椎动脉狭窄中的应用价值。方法:对本院有椎-基底动脉供血不足症状的住院及门诊患者88例(176支椎动脉)进行椎动脉彩色多普勒超声检查及数字减影血管造影(DSA)检查。以DSA检查结果为标准,评价彩色多普勒超声对椎动脉狭窄的准确性。结果:176支血管中,彩色多普勒超声检查与DSA检查结果一致的有147支(83.6%),彩色多普勒超声诊断有椎动脉狭窄而DSA诊断无狭窄的有6支(3.4%),彩色多普勒诊断无狭窄而DSA诊断有狭窄的有4支(2.2%);彩色多普勒超声判定狭窄程度高于DSA的有11支(6.3%)。彩色多普勒超声判定狭窄程度低于DSA的有8支(4.5%)。结论:彩色多普勒超声诊断椎动脉狭窄简单方便,敏感性高,可以作为一种常规检查手段,若准确评价椎动脉狭窄的程度,则需要血管造影完成。Purpose： Evaluate the application value of using color doppler ultrasonography in the diagnosis of vertebral artery stenosis. Methods： 88 cases of patients（176 branches of vertebral arteries） in both inpatient department and outpatient department with vertebrobasilar insufficiency were diagnosed through vertebral artery color doppler ultrasonography and digital subtraction angiography respectively. Take the testing result of DSA as standard, and then evaluate the veracity of applying color doppler ultrasonography in the diagnosis of vertebral artery stenosis. Results： Among 176 branches of vessels, the testing results of color doppler ultrasonography and DSA had 147 branches（83.6%） in consistent. Through color doppller ultrasonography, 6 branches（3.4%） were found vertebral artery stenosis, which DSA did not check out. However, through color doppler ultrasonography, 4 branches（2.2%） were diagnosed without stenosis, which DSA diagnosed stenosis. The stenosis degree of 11 branches（6.3%） which were diagnosed by color doppler ultrasonography is higher than that of DSA. The stenosis degree of 8 branches（4.5%） which were diagnosed by color doppler sonography is lower than that of DSA. Conclusion： The application of color doppler ultrasonography in the diagnosis of vertebral artery stenosis is simple and convenient and high sensitivity, which can be used as a routine diagnose method. If the accurate stenosis degree of vertebral artery is going to be evaluated, angiography is needed.
