皮下神经阻滞治疗带状疱疹后神经痛的疗效

摘　　要：目的观察皮下神经阻滞治疗带状疱疹后神经痛的疗效。方法带状疱疹后神经痛患者89例，应用复方倍他米松与1％利多卡因注射液在疼痛最明显的皮肤受累区行皮下神经阻滞治疗。注射后5d复诊，根据VAS评分决定是否继续治疗，记录治疗次数。比较治疗半年后不同病程与不同患病部位的疗效，观察合并病对疗效的影响。结果治疗后半年，优效率、良效率、好转率、无效率分别为65．1％（58／89例）、21．3％（19／89例）、13．4％（12／89例）、0％（0／89例），总有效率100％。不同病程、不同患病部位对疗效影响差异无统计学意义（P〉0．05）。治疗2—3次的疗效优于治疗1次的疗效（P〈0．05），治疗4～5次与2～3次的患者疗效差异无统计学意义（P〉0．05）。结论复方倍他米松与1％利多卡因注射液行皮下神经阻滞可以有效治疗带状疱疹后神经痛。Objective To observe the effects of hypodermic nerve block in the treatment of postherpetic neuralgia. Methods Eighty-nine patients with postherpetic neuralgia received hypodermic nerve block in the skin pain area with compound betamethasone and lidoeaine. Follow-up at five days after the block, whether to continue treatment and times of treatment were decided according to the VAS. The curative effect of patients in different courses and diseased areas were compared after half a year, and the influence of complicating diseases on the curative effect were observed. Results Half a year after the treatment, the excellent, good, effective effi- cacy and ineffective rate were 65.1% （58/89 cases）, 21.3% （19/89 cases）, 13.4% （12/89 cases）,0% （0/89 cases） respectively, and the total effective rate was 100%. There were no statistical difference in the curative effect in different duration and different areas （P 〉 0.05）. The curative effect with 2-3 treatment times was better than that with one time （P〈 0.05） , There was no statistical difference in curative effect between 4-5 times and 2-3 treatment times （P 〉 0.05）. Conclusion Hypodermic nerve block with compound betamethasone and lidoeaine is an effective method in the treatment of postherpetic neuralgia.
