关节镜下行微创无头加压螺钉固定在治疗髌骨横型骨折中的价值分析

摘　　要：目的探讨关节镜下行微创无头加压螺钉固定在治疗髌骨横型骨折中的价值。方法随机选择在该院经影像诊断为髌骨横行骨折的患者100例,随机分成观察组和对照组,分别微创无头加压螺钉及空心钉治疗,比较两组患者的临床指标、和健侧对比的活动度损失、Lysholm评分以及不良反应对。结果观察组患者的手术时间显著短于对照组(t=9.586,P=0.004),观察组患者和健侧比活动度的损失显著小于对照组患者(t=-12.542,P=0.000),Lysholm评分显著高于对照组患者(t=7.635,P=0.014),两组结果差异有统计学意义(P<0.05);观察组术后不良反应率为2%,对照组不良反应率为24%,两组结果差异有统计学意义(χ2=21.397,P<0.05)。结论关节镜下行微创无头加压螺钉固定治疗髌骨横型骨折具有术后不良反应少、临床效果良好等优势,是高效便捷的治疗手段,值得在临床推广。Objective To study the value of arthroscopic minimally invasive acutrak compression screw fixation in treatment of patella transverse fractures. Methods 100 cases of patients with patella transverse fractures diagnosed by imaging in our hospital were selected and randomly divided into two groups, the observation group were treated with minimally invasive acutrak compression screw, the control group were treated with cannulated compression screw, and the clinical index, the motion range loss compared with that of the uninjured side, Lysholm score and adverse reactions were compared between the two groups. Results The operative time in the observation group was obviously shorter than that in the control group,（t=9.586, P=0.004）, the motion range loss compared with that of the uninjured side in the observation group was obviously less than that in the control group,（t=-12.542, P =0.000）and the Lysholm score in the observation group was obviously higher than that in the control group（t=7.635, P=0.014）, and the difference was obvious,（P〈0.05）, and the difference in the postoperative adverse reaction rate between the observation group and the control group was obvious,（2% vs 24%）, with statistical significance,（χ2=21.397, P〈0.05）. Conclusion Arthroscopic minimally invasive acutrak compression screw fixation in treatment of patella transverse fractures has fewer postoperative adverse reactions and a good clinical effect, and it is an efficient and convenient treatment method, which is worth clinical promotion.
