PLIF围手术期隐性失血相关影响因素初步分析

摘　　要：目的初步探讨分析腰椎后路椎体间植骨融合术(posterior lumbar interbody fusion,PLIF)围手术期患者一般情况与隐性失血间的相关性。方法回顾2012年8月至2014年12月在我院接受PLIF手术且临床资料完整的42例病例,统计分析患者性别、年龄、BMI、ASA分级、手术节段、手术时间以及术中、术后失血情况,计算隐性失血量。通过单因素方差及多元线性回归模型分析患者围手术期一般情况对于隐性失血的影响。结果单因素分析显示手术节段组与手术时间组内隐性出血量均存在统计学差异(P<0.05)。多变量线性回归模型提示手术节段是影响隐性出血的独立危险因素。单节段手术组隐性出血量明显少于双节段及多节段手术组(P<0.05),但三者隐性失血量占总失血量比例无明显差别。结论 PLIF手术时间延长可能会增加围手术期隐性失血量,手术节段是影响围手术期隐性出血的危险因素之一。Objective To preliminarily analyze the correlation between the hidden blood loss and perioperative factors in patients received posterior lumbar intervertebral fusion（ PLIF）. Methods We retrospectively analyzed 42 patients underwent PLIF surgery in our hospital. The collection date were grouped according to perioperative factors such as gender,age,BMI,ASA grading,surgery level and operation time. And we evaluated the influence of perioperative factors on hidden blood loss by single factor variance analysis and multivariate linear regression. Results The single factor analysis showed the differences of hidden blood loss were statistically significant in the surgery level group and surgery time group（ P〈0. 05）. Multivariate linear regression model show surgery level was an independent risk factor influencing the hidden blood loss. The hidden blood loss in single level surgery group significantly less than double levels surgery group and multiple level surgery group（ P〈0. 05）.However,no obvious difference in ratio of hidden blood loss to total blood loss was observed among different surgery level group.（ P〈0. 05）. Conclusion More surgery time may associate with more hidden blood loss. Surgery levels is one of the independent factors affected hidden blood loss in patients received PLIF.
