摘 要:目的:探究胫骨中段横断骨折内外侧锁定钢板固定的生物力学比较。方法:收集8具完整防腐成年尸体下肢标本,依据左右腿配对按照随机数字分配法,分为内侧锁定钢板固定组(内侧组)和外侧锁定钢板固定组(外侧组),于胫骨骨折线共同水平面上锯断腓骨,进行四点弯曲、轴向压缩实验以及扭转实验,对内外侧锁定钢板固定的生物力学进行比较。结果:与外侧组相比,500N外力下位移较小,差异具有统计学意义(P<0.05);900N外力下位移较小,差异具有统计学意义(P<0.05);15度扭转角度下扭矩较大,差异具有统计学意义(P<0.05);腓骨骨折与完整腓骨相比,500N外力下位移较大,差异具有统计学意义(P<0.05);900N外力下位移较大,差异具有统计学意义(P<0.05);15度扭转角度下扭矩较小,差异具有统计学意义(P<0.05)。结论:胫骨中段横断骨折中内侧钢板固定较外侧钢板固定稳定性提高,腓骨的完整程度与钢板稳定性呈正比,对稳定性具有增强作用。Objective: To investigate the biomechanical comparison in internal fixation of lateral locking plate fixation for tibial fractures. Methods: 8 intact antisepetic adult lower limb specimens were selected and divided into the medial locking plate fixation group( medial group) and the lateral locking plate fixation group( lateral group). On the fracture line of the tibia,the fibula was cut from the horizontal plane,and the four point bending,axial compression test and torsion test were performed and the biomechanics of internal and external fixation of locking plate were compared. Results: Compared with the lateral group,the 500 N external force displacement were lower( P〈0.05),the 900 N external force displacement were lover,the 15° degree torsion torque were higher( P〈0.05). Compared with the intact fibula,the 500 N external force displacement were higher( P〈0.05),the 900 N external force displacement were higher( P〈0.05),the 15° degree torsion torque were lower( P〈0.05). Conclusion: The stability of medial lateral plate fixation in the treatment of the middle and medial transverse fractures of the tibia is increased,and the integrity of the fibula is proportional to the stability of the steel plate.
