钻孔减压自体腓骨支撑治疗股骨头坏死的临床疗效观察

摘　　要：目的:探讨观察钻孔减压自体腓骨支撑治疗股骨头坏死(Osteonecrosis of the Femeral Head,ONFH)的临床疗效并作出评价。方法:选取2010年2月至2014年12月期间,我院收治的股骨头坏死Ⅱ期、Ⅲ期(Ⅲa和和Ⅲb)患者108例作为研究对象,采用钻孔减压自体腓骨支撑治疗,观察患者临床疗效,包括术中出血量、术后恢复(采用Harris评分)及治疗12个月后影像学检查情况等并作出评价。结果:Ⅱ期、Ⅲ期患者术中出血量无明显差异,Ⅱ期患者术后12个月影像学检查恢复情况较佳,Ⅱ期、Ⅲ期总改善率分别为(95.3%)、(66.7%),Ⅱ期疗效优于Ⅲ期,(P<0.05)差异有统计学意义。Ⅱ期、Ⅲ期患者Harris评分治疗前无明显差异,总优良率分别为(17.6%)和(20.0%);治疗后较治疗前有显著提高,分别为(94.1%)和(67.5%),组内和组间比较差异均有统计学意义(P<0.05)。,结论:钻孔减压自体腓骨支撑治疗ONFH的临床疗效较佳,能够有效控制塌陷的进展,Ⅱ期疗效优良率高,恢复情况较好,减轻了患者的痛苦,具有较高的临床应用价值。Objective： To discuss and observe drilling decompression Autogenous fibular support treat osteonecrosis of the femeral head（ ONFH） clinical curative effect and make a remark.Methods： A total of 108 patients with Osteonecrosis of the Femeral Head from February 2010 to December 2014 from our hospital were divided into Ⅱperiod and Ⅲ period（ Ⅲa and Ⅲb period） according to course of disease as research object. Adopt drilling decompression autogenous fibular strut treatment to treat,observing patient clinical therapeutic effects,including blood loss during operation,recovery after treatment（ adopt Harris score as gist） and imageological examination after treatment 12 months and make a remark.Results： Ⅱperiod patient compared withⅢ period patient in blood loss during operation,difference was no statistical significance（ P〈0.05）. Ⅱperiod patient imageological examination after treatment 12 months was excellent,Ⅱperiod and Ⅲ period patient total improvement rate respectively was（ 95.3%） and 66.7%）,Ⅱperiod effect better than Ⅲ period effect,differences were statistical significance（ P〈0. 05）. Ⅱ period patient compared with Ⅲ period patient in Harris score before treatment was no statistical significance（ P〈0.05）,respectively was（ 17.6%） and（ 20.0%）.After treatment the score were improved obviously,total effective rate respectively was（ 94.1%） and（ 67.5%）,comparison between group and group differences were statistically significant（ P〈0.05）. Conclusion： Drilling decompression Autogenous fibular support treat osteonecrosis of the femeral head has great clinical curative effect,can effectively control collapse situation,Ⅱperiod clinical effect is excellent,recovery conditions are good,reduced patients＇ pain and has high clinical value.
