64排螺旋CT容积重建及多平面重建技术对SPS诊断的方法探讨

摘  要:目的:提高64排螺旋CT容积重建(volume rendering,VR)、多平面重建(multi-plannar reconstruction,MPR)技术对茎突过长综合征(styloid process sundrome,SPS)诊断的方法认识。方法:对56例临床查体疑诊茎突过长的患者行传统CT检查,组成传统CT检查组;并行64排螺旋CT扫描加MPR、VR重建检查,组成64排螺旋CT扫描组。检查项目为观察、测量茎突(包括茎突增长、中部不连及倾斜角度改变)变化;并将检查结果进行分析对比。结果:42例茎突过长病例,以茎突长度大于3.0 cm为诊断标准,64排螺旋CT组阳性检出率为85.71%,传统CT检查组阳性检出率为66.67%;14例倾斜角过大病例,64排螺旋CT组阳性检出率为85.71%,传统CT检查组阳性检出率为57.14%;组间均有显著性统计学差异(P<0.01)。结论:64排螺旋CT容积重建能较形象地显示三维立体图像,直观显示茎突外貌、测量茎突倾斜角度,多平面重建技术对测量茎突长度较传统X线及传统CT冠位更准确,在对茎突过长综合征诊断中值得推广。Objective To improve volume rendering(VR) and multiplanar reconstruction(MPR) of 64-row helical CT for the diagnosis of styloid process syndrome(SPS).Methods Totally 56 cases suspected with SPS underwent conventional CT examination for growth,intermediate non-union overturn angle of the styloid process,and the results were enrolled into a conventional CT group,and the results of VR and MPR reconstruction for the 56 ones were involved in a 64-row CT group.The results in the two groups were compared.Results The diagnostic criteria was set as the styloid process length more than 3.0 cm.For the 42 patients with elongated styloid process,the detection rate by 64-row CT was 85.71%,and the rate by conventional CT was 66.67%.For the 14 ones with enlarged overturn angle,the detection rate by 64-row CT was 85.71%,and the rate by conventional CT was 57.14%.There were statistical differences between all the data of the groups(P〈0.01).Conclusion 64-row helical CT had VR behave better in diagnosing elongated styloid process and MPR in measuring the length of the styloid process when compared with conventional CT,and thus is worthy popularizing for the diagnosis of SPS.
