摘  要:目的分析医院处方点评中存在的典型问题及有针对性地提出建议,促进临床合理用药。方法采用回顾性分析方法,统计2013年7月至2014年12月的处方点评结果,总结出典型问题,分析原因并提出改进措施。结果 46 009张处方中,不合格处方448张,占0.97%。不合格类型主要为临床诊断未写或书写不全(25.22%),用法、用量不适宜(25.67%),诊断与用药不符(21.21%),联合用药不适宜(10.27%)。结论通过总结处方点评典型问题并提出改进方法,达到了持续改进处方质量的目的,建立了可行的处方点评工作模式,培养了年轻临床药师,但处方点评仍存在一定问题,有待进一步改善。Objective To analyze and solve the typical problems in the hospital prescription review with feasible work mode,and to pro- mote the clinical rational drug use. Methods The retrospective analysis method and statistical analysis were used on the results of pre- scription review from July 2013 to December 2014;the typical problems were summed up and analysis of the reasons for the improved method was conducted. Results In the 46 009 prescriptions,448 pnescriptions were unreasonable, accounting for 0. 97%. The types of unreasonable prescription mainly were: no clinical diagnosis or clinical diagnosis not complete(25.22% ),inappropriate usage and dosage (25.67%) ,inconsistent diagnosis and medication(21.21% ) ,inappropriate combination(10.27% ). Conclusion Through summarizing typi- cal problems of the prescription review and put forward improvement methods ,the goal of the of continuous prescription quality improve- ment is achieved, the feasible prescription review work mode is set up, young clinical pharmacists are trained. But there are still some problems on the prescription review which needs to be further improved.
