非典型性脑膜瘤的磁共振成像表现

摘　　要：目的探讨非典型性脑膜瘤的磁共振成像(MRI)表现。方法回顾性分析经手术病理证实的21例非典型性脑膜瘤的MRI表现。结果本组21例非典型性脑膜瘤,部位典型者13例,典型部位主要位于大脑凸面、大脑镰和桥小脑角区;非典型者8例,其中颅内外沟通5例,侧脑室3例。MRI扫描显示,肿瘤呈类圆形7例,分叶状5例,不规则形9例。T1WI低信号15例,等信号6例;T2WI稍高信号13例,等高混杂信号8例;增强后不均匀明显强化14例,均匀明显强化7例。结论非典型性脑膜瘤大多具有特征性的MRI表现。Objective To analyze the MR features of atypical meningiomas , to improve the diagnostic accuracy of this tumor presurgery. Methods MRI data of 21 patients with atypical meningioma proved by surgery and pathol-ogy were retrospectively analyzed. Results Of 21 cases, 13 were located in typical places, mainly at the convexity of brain, cerebral falx, and cerebellopontine angle. Of 8 cases with atypical location, intra-extracranial communication meningioma in 5 cases and lateral ventricle in 3 cases. By MRI, 7 cases showed heterogeneous intensity, 5 cases had lobular form, 9 had irregular shape. The lesions were demonstrated hypointensity in 15 cases, isointensity in 6 cases by T1WI, while slightly hyperintensity in 13 cases, mixed signal in 8 cases by T 2WI. After Gd-DTPA injection, obvious inhomogeneous enhancement were showed in 14 cases while obvious homogeneous enhancement in 7 cases. Conclu-sion The most of atypical meningiomas were of characteristics on MRI.
