摘　　要：目的探讨臭氧大自血联合CT引导下连续腰交感神经阻滞治疗血栓闭塞性脉管炎(thromboagiitis oblit-erans,TAO)的临床疗效、安全性及作用优势。方法选取2012年5月至2015年9月解放军总医院收治的有疼痛症状的TAO患者作为研究对象。采用随机数字表法分为对照组和试验组。对照组给予单纯CT引导下连续腰交感神经阻滞治疗,试验组给予臭氧大自血联合CT引导下连续腰交感神经阻滞治疗。比较2组患者视觉模拟评分(VAS)、疼痛缓解时间及肢体缺血程度分期变化等指标。结果 38例患者纳入本研究,每组19例。2组患者一般情况比较,差异无统计学意义。各组治疗后不同时点与治疗前的VAS评分比较,差异均有统计学意义(P<0.05);2组间同时点比较,治疗第3天的差异无统计学意义(P>0.05),治疗第10、30天试验组VAS评分较对照组明显降低,差异有统计学意义(P<0.05)。结论臭氧大自血联合CT引导下连续腰交感神经阻滞治疗可以改善末梢血运循环、增加体内氧供,缓解TAO患者的疼痛。Objective To evaluate the safety,effectiveness and the advantages of ozone autohemotherapy combined with continuous lumbar sympathetic block guided with CT in treatment of patients with lower extremity thrombosis obliterans.Methods The thrombosis obliterans patients who were admitted to the hospital from May 2012 to September2015 for pain were recruited and randomly divided into the control group and the treatment group.The control group were treated by simple CT-guided continuous lumbar sympathetic block therapy,while the treatment group were applied ozone autohemotherapy combined with CT-guided continuous lumbar sympathetic block therapy.All the patients were analyzed in terms of the VAS scores,pain remission time,changes in the degree of limb ischemia stage and other aspects.Results A total of 38 patients were identified.The demographic data were similar in two groups.There was significant difference in the VAS scores between before and after treatment in two groups（P〈0.05）.There was no difference（P〉0.05）of the VAS on the third day between two groups.There were significant differences of the pain scores,the degree of limb ischemia stage（P〈0.05）on the tenth day and the thirtieth day.Conclusion Ozone autohemotherapy combined with CT-guided continuous lumbar sympathetic block can improve peripheral blood circulation,increase in vivo oxygen supply,relieve pain in thrombosis obliterans patients.
