摘　　要：目的观察经皮穴位电刺激(transcutaneous acupoint electrical stimulation,TAES)对前扣带回(anteriorcingulated cortex,ACC)与全脑功能连接的影响,探讨ACC在TAES中枢镇痛机制中的作用。方法选择健康受试者14例(男7例,女7例),依次在基础阶段和TAES 30 min后行功能磁共振扫描。选取ACC为感兴趣脑区,观察TAES后ACC与全脑功能连接的改变。结果 TAES增强了ACC与双侧小脑、额叶内侧、额叶眶回、额上回、额中回、顶下小叶的功能连接,减弱了ACC与后扣带回、双侧楔前叶及楔叶的功能连接(P<0.05)。结论 TAES改变了ACC与广泛镇痛相关脑区间的联系,并通过ACC从情感、疼痛下行调节系统、疼痛预期、疼痛感觉认知以及记忆等方面对疼痛进行调节。ACC在TAES镇痛中起到了重要的作用。Objective To observe the changes of functional connectivity of anterior cingulated cortex（ACC）induced by transcutaneous acupoint electrical stimulation（TAES）and explore the role of ACC in central mechanism of analgesia induced by TAES.Methods Fourteen（7 male,7 female）healthy right-handed volunteers received functional magnetic resonance imaging（f MRI）scan before and after TAES respectively.ACC was selected as the region of interest（ROI）.The functional connectivity between ACC and the whole brain was observed.Results Compared with the basic state,TAES significantly increased the functional connectivity of ACC with bilateral regions of cerebella,the medial frontal lobe,the orbital gyrus of the frontal,the superior gyrus of the frontal,the middle gyrus of the frontal,the inferior parietal lobe,while significantly reduced the functional connectivity of ACC with posterior cingulated cortex（PCC）,the bilateral regions of precuneus gyrus and cuneus gyrus.Conclusion TAES changes the relationship between ACC and massive analgesic related brain regions.TAES relieves pain by adjusting the pain related emotion,the descending regulation system of pain,pain expectation and the long term memory through ACC.ACC may play a key role in central analgesia induced by TAES.
