白芍总苷联合得宝松局部注射治疗斑秃的疗效观察及其对血清TNF-γ和IL-10的影响

摘　　要：目的研究白芍总苷联合得宝松局部注射治疗斑秃的疗效及其对外周血干扰素-γ(TNF-γ)和白细胞介素-10(IL-10)的影响。方法选取2012年8月—2015年8月入院诊治的斑秃患者120例,随机分为试验组(60例)和对照组(60例)。对照组单纯局部注射得宝松,试验组在其基础上联用白芍总苷,2组均治疗8周为1个疗程。疗程结束后首周周一,观察2组患者的治疗效果,观察并比较2组患者治疗前后的斑秃严重程度、血清TNF-γ和IL-10水平以及不良反应发生情况。结果试验组痊愈44例,显效9例,总有效率88.33%;对照组痊愈25例,显效19例,总有效率73.33%。2组差异有统计学意义(P<0.05)。治疗前2组患者的TNF-γ和IL-10差异无统计学意义,经治疗后患者TNF-γ水平下降、IL-10升高,且以试验组差异更为显著(P<0.05)。治疗前2组患者斑秃严重程度评分(SALT评分)差异无统计学意义,治疗后患者SALT评分均有所下降,以试验组下降更为显著(P<0.05)。结论应用白芍总苷联合得宝松局部注射治疗斑秃疗效确切,可显著降低SALT评分,并且可调控辅助性T淋巴细胞1型/2型细胞因子平衡状态。Objective In order to investigate efficacy and serum TNF-γ IL-10 of patients with alopecia areata that treated with TGP glucosides and Diprospan local injection. Methods One hundred and twenty alopecia areata patients from August 2012 to August 2015 were selected, and they were randomly divided into trial group （60） and control group （60）. Control group were treated with injecting Diprospan partially, and trial group patients were treated with injecting Diprospan and TGP partially. Two groups were treated for eight weeks as a course of treatment. After treated for the first week on Monday, the treatment effect was observed in the two groups, and alopecia areata severity of serum TNF-γ, IL-10 levels and adverse reactions of two patients were observed and compared. Results Forty-four cases were cured, 9 case were markedly take efficacy, total efficiency rate was 88.33% in trial group; 25 cases were cured, 19 case were markedly take efficacy, total efficiency rate was 73.33% in control group, total efficiency rate of trial group was higher than control group （P 〈0.05）. Before treatment, two group patients＇ TNF-γ and IL-10 had no significant difference. After treatment, two group patients＂ TNF-% IL-10 were increased, and trial group were higher than control group （P〈0.05）. Before treatment, two group patients; alopecia areata severity score （SALT score） had no significant difference. After the treatment, two group patients; TNF-γ and IL-10 were decreased, and trial group were lower than control group （P〈0.05）. Conclusion TGP glucosides combined with Diprospan local injection in treating alopecia areata can significantly reduce the SALT rates, and also can adjust the type oft helper type 1/2 cytokine balance.
