摘 要:目的探讨舒肌汤治疗帕金森病肌僵直的可能作用机理。方法采用颅内注射乳胞素诱导帕金森病肌僵直大鼠模型共24只,随机分为模型组和中药高、中、低剂量组,每组6只,另设假手术组6只(颅内注射生理盐水)。假手术组和模型组均灌胃生理盐水20 g/(kg·d),舒肌汤浓缩至2.0 g/ml,中药高、中、低剂量组分别灌服舒肌汤40、20、10 g/(kg·d),每日1次,持续灌胃30天。检测大鼠纹状体多巴胺(DA)、高香草酸(HVA)和二羟苯乙酸(DOPAC)的含量,电镜观察膈肌神经肌肉接头超微结构改变。结果与假手术组比较,模型组大鼠纹状体DA、HVA、DOPAC含量明显减少,DA/DOPAC和DA/HVA值下降(P<0.05或P<0.01);与模型组比较,中药中、低剂量组大鼠纹状体DA、HVA、DOPAC含量及DA/DOPAC和DA/HVA值均升高(P<0.05或P<0.01);与中药中剂量组比较,中药高剂量组DOPAC含量和DA/DOPAC、DA/HVA值下降,中药低剂量组DOPAC含量亦降低(P<0.05)。电镜下,模型组存在神经肌肉接头处肌纤维密集,排列紊乱,突触前膜囊泡和线粒体增多,突触后膜皱褶明显,乙酰胆碱受体增多等超微结构改变,中药各剂量组均能不同程度改善上述改变。结论舒肌汤可能通过促进DA分泌,同时减少神经肌肉接头处乙酰胆碱的释放,调节中枢和外周神经兴奋,从而缓解帕金森病肌僵直。Objective To explore possible functional mechanism of Shuji Decoction (舒肌汤) in treating Parkinson disease muscle rigidity. Methods Using encephalic injection of Lactacystin to induce 24 Parkinson disease rigidity model rats in total. Divided them into the model group and the Chinese medicine high, middle and low dose groups randomly, with 6 in each group. Set 6 rats in the sham-operation group in addition( injected with saline encephalicly). The sham-operation group and the model group were both perfused with saline 20 g/( kg · d). Shuji Decoction was concentrated to 2.0 g/ml. The Chinese medicine high, middle and low dose groups were respectively gavaged with Shuji Decoction 40, 20 and 10 g/( kg ~ d), once per day, for 30 days. Examined content of dopamine (DA), hom- vanillic acid hva (HVA) and dihydroxyphenylaceticacid (DOPAC) in corpus striatum. Observed changes of ultrastructure at neuromuscular junction of nervus phrenicus under electron microscopy. Results Compared with the sham-operation group, the content of DA, HVA and DOPAC in rats of the model group remarkably decreased, with value of DA/DOPAC and DA/HVA decreased (P 〈0. 05 or P 〈0. 01 ) ; Compared with the model group, the content of DA, HVA, DOPAC, value of DA/DOPAC and DA/HVA of corpus striatum of mice in the Chinese medicine high, middle and low dose groups all remarkably increased (P 〈0. 05 or P 〈0.01 ) ; Compared with the Chinese medicine middle dose group, content of COPAC, and value of DA/DOPAC, DA/HVA of the Chinese medicine high dose group decreased, and content of DOPAC of the Chinese medicine low dose group also decreased (P 〈 0. 05). Under elec- tron microscopy, there existed changes of uhrastructure like muscular fiber concentrating and being arranged in disorder at neuromuscular junction, vesicles and mitochondria of presynaptic membrane increasing, postsynaptic membrane folding remarkably and acetylcholine receptors increaing.
