不同干扰素体外释放酶联免疫试剂在结核病诊断中的应用价值比较

摘　　要：目的探讨国产结核分枝杆菌相关干扰素体外释放酶联免疫法试剂盒(TB-IGRA)在结核病诊断中的应用价值。方法用国产的TB-IGRA试剂盒分别检测227例结核病患者(包括191例肺结核和36例肺外结核)、149例非结核检测对象(包括99例非结核肺部疾病患者和50例健康对照者)外周血特异性γ-干扰素的含量,同时与进口TSPOT.TB试剂进行平行比较分析。结果 TB-IGRA检测结核病患者阳性率为88.1%,TB-IGRA试剂检测肺结核和肺外结核的阳性率差异无统计学意义(P>0.05);TB-IGRA检测方法的特异度为81.9%,其他肺部疾病患者和正常对照组特异性差异无统计学意义(P>0.05)。TB-IGRA和T-SPOT.TB检测方法的灵敏度和特异度差异无统计学意义(P>0.05)。结论 TB-IGRA检测肺内、肺外结核具有较高的灵敏度和特异度,值得临床推广应用。Objective To evaluate the diagnosis value of domestic made T- cell based IFN- γ release assays（ TB- IGRA） in the diagnosis of tuberculosis. Methods We detected the specific IFN- γ content in peripheral blood from 227 cases of tuberculosis（ included 191 pulmonary TB and 36 extrapulmonary TB） and 149 cases of non- tuberculosis（ including 99 patients with other respiratory diseases and 50 healthy controls） with domestic made TB- IGRA,and at the meantime,we did the same detections with imported T- SPOT. TB regents. The results were compared between the two groups. Results The positive rate of TB patients was 88. 1% with TB- IGRA,and there was no statistical significance on the differences of the positive rate between in the detection of pulmonary tuberculosis and in the detection of extrapulmonary tuberculosis（ P〉0. 05）. The specificity of TB- IGRA was 81. 9%,and there was no statistical significance on the specificity differences between the detection of other respiratory diseases and healthy controls group（ P〉0. 05）. The differences of sensitivity and specificity with TB- IGRA and T-SPOT. TB were not statistically significant（ P〉0. 05）. Conclusion TB- IGRA is a sensitive and specificity method for the detection of pulmonary TB and extrapulmonary TB. It is worthy of being developed and adopted in clinical practice.
