摘 要:目的研究随机尿IgG与尿肌酐比值(IgG/Cr)是否可以代替24 h尿蛋白定量(UPRO),用于子痫前期的诊断和病情监测。方法选取2013年5月-2015年5月在温州市中心医院收治的轻度子痫前期孕妇31例,重度子痫前期孕妇33例,正常孕妇25例,留取24 h尿液检测24 h尿蛋白,随机尿液检测尿白蛋白、尿IgG、尿肌酐;尿白蛋白/尿肌酐(A/Cr)、尿IgG/Cr和UPRO进行相关性分析。采取ROC曲线分析确定随机尿IgG/Cr相对应UPRO为0.3 g及5 g时的诊断界点。结果随机尿A/Cr、IgG/Cr同UPRO呈正相关(r值分别为0.646和0.872,P<0.01);随机尿IgG/Cr相对应UPRO为0.3 g及5 g时的诊断界点分别为15.6 mg/g、375.2 mg/g时,曲线下面积分别为0.963和0.917。结论随机尿IgG/Cr同24 h尿蛋白呈显著正相关,优于随机尿A/Cr,用于子痫前期诊断较24 h尿蛋白更为快捷和简便,有望成为子痫前期诊断指标。Objective To study whether random urinary IgG and urinary creatinine ratio(IgG/Cr) can be used to replace the 24 h urinary protein quantitative(UPRO) for the diagnosis of preeclampsia and disease monitoring. Methods 31 women with mild preeclampsia, 33 women with severe preeclampsia and 25 normal pregnant women at the Wenzhou Central Hospital during May 2013 - May 2015 were recruited. To collect 24 - hour urine for UPRO, and to detect albumin, immunoglobulin G and creatinine in samples. The correlations among urine albumin/ereatinine(A/Cr) , urine IgG/Cr and UPRO were conducted for correlation analysis. ROC curve analysis was adopted to determine the diagnostic boundary points of random urine IgG/Cr corresponding to UPRO of 0.3 g and 5 g. Results A/Cr, IgG/Cr and UPRO were positively correlated, with the correlation coefficients(r) of 0.646 and 0.872( P 〈0.01 ) respectively. By the ROC curve analysis, the IgO/Cr of 15.6 mg/g and 375.2 mg/g in urine samples represented the most appropriate threshold for detecting preeclampsia with UPRO of 0.3 g and 5 g, with their area under the ROC curve of 0. 963 and 0.917 respectively. Conclusion Random urinary IgG/Cr and 24 h urinary protein were significantly positively correlated, which is better than random urine A/Cr, and it is more rapid and convenient for the diagnosis of preeclampsia compared with 24 h urine protein, and is expected to become the diagnosis index of preeelampsia.
