摘  要:目的探讨荨麻疹患者血清趋化因子CCL11、血浆D-二聚体水平变化及其与疾病发生发展之间的关系。方法选择51例急性荨麻疹(Acute Urticaria,AU)、慢性荨麻疹(Chronic Urticaria,CU)患者,根据病情分为轻、中、重度组,并挑选健康成人作为正常对照组。采用酶联免疫吸附试验检测3组荨麻疹患者以及正常对照组血清CCL11及血浆D-二聚体水平,并分析其与荨麻疹症状评分(Urticaria Activity Score,UAS)的相关性。结果 CCL11在轻度、中度和重度荨麻疹中的水平分别是20.12(15.40-23.76)、23.85(11.70-28.25)、18.84(13.10-39.31),组间比较差异无统计学意义(P=0.95)。D-二聚体在轻度、中度和重度荨麻疹中的水平分别为935.00(580.00-3 360.00)、2 225.00(850.00-65510.00)、3 865.00(1 235.00-7 820.00),重度和轻度组间比较差异有统计学意义(P=0.04),血清CCL11水平与D-二聚体未见显著相关(r=-0.078,P=0.594)。结论趋化因子CCL11、D-二聚体水平在急、慢性荨麻疹均升高,提示两者可能参与疾病发生,且D-二聚体升高提示疾病活动,可作为病情的监测指标。Objective To determine the changes of serum levels of chemokine CCL11 and D-dimer and investigate theirrelationship with the disease progress in the urticaria patients. Methods A total of 51 patients with acute urticaria(AU)orchronic urticaria(CU)admitted in our department were enrolled in the study. They were divided into 3 groups according to theseverity of the disease,that is,mild,moderate,severe groups. Another 25 healthy adults served as normal control group. Serumlevels of CCL11 and D-dimer were measured by ELISA,and the correlation of the levels of CCL11 and D-Dimer with urticariaactivity score(UAS)were analyzed. Results The serum CCL11 level was 18.84(13.10-39.31),23.85(11.70-28.25)and20.12(15.40-23.76)pg/m L respectively,in the mild,moderate,severe groups,and there was no significant difference amongthe groups(P=0.95). And the D-dimer level was 3 865.00(1 235.00-7 820.00),2 225.00(850.00-65 510.00)and 935.00(580.00-3 360.00)μg/L respectively in the 3 groups,and significance difference was found between the mild and severe groups(P=0.04). There was no significant correlation was seen between the serum levels of CCL11 and D-dimer( r =0.078,P=0.594). Conclusion Both chemokines CCL11 and D-dimer are elevated in AU and CU patients,indicating that both of themmay be involved in the occurrence of the disease. What's more,elevated D-dimer level suggests the disease activity,and canbe used as monitoring indicator for the disease.
