中国老年人健康期望寿命及其影响因素研究

摘　　要：目的了解中国老年人的健康期望寿命现状,分析老年人健康期望寿命的影响因素。方法以＂中国老年人口健康长寿调查研究＂（CLHLS）2005-2011年的三次纵向调查资料为基础,通过多状态生命表法计算老年人的自理期望寿命和自评健康期望寿命;采用二次多项式模型,分地区进行曲线拟合;采用简单线性回归和多重线性回归方法分析健康期望寿命的影响因素。结果共纳入研究对象11658例,老年人的自理期望寿命和自评健康期望寿命均以65-71岁年龄段最高,分别为12.25年和10.48年;中部地区老年人的健康期望寿命相对处于弱势;年龄、性别、地区、民族、婚姻状况、居住方式、文化程度、生活方式及有无慢性病是健康期望寿命的主要影响因素。结论我国老年人的健康期望寿命值得关注,加强对高龄、男性、中部、丧偶、独居、文化程度低和患慢性病等老年人群的健康管理,提倡健康的生活方式,应成为今后健康老龄化工作的重点。Objective To investigate the healthy life expectancy of elderly and explore its impact factors in China.Methods A longitudinal survey data of Chinese Longitudinal Healthy Longevity Survey（ CLHLS） w ere used. It w as calculated that the Active Life Expectancy and Self rated health life expectancy of elderly using the multistate life table method. Quadratic polynomial models w ere estimated for healthy life expectancy curve by area. Linear regressions w ere used to analyze the impact factors of healthy life expectancy. Results 11,658 cases w ere included in the study. Active Life Expectancy and Self rated health life expectancy of elderly w ere highest in 65 ~ 71 years of age,w hich are 12. 25 years and 10. 48 years,respectively. The elderly health life expectancy of central region is low er than elsew here. Age,gender,area,ethnicity,marital status,living arrangements,education level,lifestyle and chronic diseases w ere significant influencing factors of the healthy life expectancy. Conclusion The healthy life expectancy of elderly required urgent attention. Strengthening the health management for the elderly w ho w ere older,men,w idow ed,living alone,low education and suffering from chronic diseases etc.,and promoting healthy lifestylesshould be the w ork focus of health ageing.
