不同剂量地佐辛预处理减轻丙泊酚注射痛的临床研究

摘　　要：目的观察麻醉诱导前不同剂量地佐辛预处理减轻丙泊酚注射痛的临床效果。方法择期全麻手术患者120例,随机分为地佐辛高剂量组(DG组)、中剂量组(DZ组)、低剂量组(DD组)及生理盐水组(NS组),每组30例。麻醉诱导前DG组、DZ组、DD组分别静注地佐辛0.2、0.1、0.05 mg/kg,NS组静注生理盐水5 m L,15 min后各组静注丙泊酚1 mg/kg。记录四组患者丙泊酚注射痛、静注丙泊酚前不良反应(呼吸抑制、恶心呕吐及头晕)的发生率及静注丙泊酚前(T0)、静注丙泊酚后(T1)的平均动脉压(MAP)和心率(HR)的变化。结果与NS组、DD组比较,DG组、DZ组注射痛发生率降低(P<0.05);与T0时比较,T1时四组患者的MAP和HR均降低(P<0.05);DG组不良反应生率均高于其他组(P<0.05);DG组、DZ组注射痛发生率,DZ组、DD组、NS组不良反应发生率,及T0、T1时四组MAP和HR比较差异均无统计学意义(P>0.05)。结论高、中剂量地佐辛预处理均可有效减轻丙泊酚注射痛,中剂量相对理想。Objective To observe the clinical effect of pretreatment with different doses of dezocine to reduce injection pain of propofol before induction of anesthesia.Methods One hundred and twenty patients undergoing elective surgery were randomly divided into four groups（30 cases in each group）：high-dose dezocine group（group DG）,middle-dose dezocine group（group DZ）,low-dose dezocine group（group DD） and normal saline group（group NS）.Before induction of anesthesia group DG,group DZ and group DD respectively received intravenous injection of dezocine at 0.2,0.1 and 0.05 mg/kg;group NS received intravenous injection of 5 mL normal saline.Fifteen min later,four groups received intravenous injection of 1 mg/kg propofol.The injection pain of propofol,side effects before propofol intravenous administration（respiratory depression,nausea and vomiting and dizziness） and the changes of mean arterial pressure（MAP） and heart rate（HR） before propofol intravenous administration（T_0） and after propofol intravenous administration（T_1） were recorded.Results Compared with group NS and group DD,the incidence of injection pain was lower in group DG and group DZ（P〈 0.05）.Compared with T_0,the MAP and HR were lower at T_1in four groups（P〈0.05）.The incidence of adverse effects in group DG was higher than that of the other groups（P〈0.05）.There was no significant difference in the incidence of injection pain between group DG and group DZ,and no significant difference was found in the incidence of adverse effects among group DZ group DD and group NS,and the MAP and HR at T_0 and T_1 among the 4 groups（P〉0.05）.Conclusion Pretreatment with high-dose dezocine or middle-dose dezocine can effectively reduce the injection pain of propofol,and middle dose is relatively better.
