基于抑郁症临床诊断标准的病机探讨

摘　　要：抑郁症是全球第四大疾病负担,也是导致患者功能残疾的主要原因之一,大约有1/7的人会在人生的某个阶段遭受抑郁症困扰。中医治疗本病历来多从肝论治,认为其核心病机在于肝气郁结。文章通过抑郁症诊断标准中的临床表现,结合中医传统理论,认为虽然其核心病机在于肝气郁结,但阳气虚衰应为抑郁症的根本病机,肝气虚导致的肝主疏泄不及、肝气郁结为标,多情致病伤及肝气和肾阳亏虚推动无力导致的肝气虚衰为本。强调在中医辨治抑郁症时除需疏肝解郁、条畅气机外,亦需重视振奋肾阳、补益肝气,为临床辨治抑郁症提供新思路,以期更好地改善抑郁诸病,缓解患者身心痛苦。Depression is the fourth disease which affects patients＇ lives seriously and leads to functional impairment of body, and affects 1/7 of population in the world. It is always treated according with the liver from traditional Chinese medicine and its core pathogenesis is thought as deficiency of liver qi. Through the clinical manifestation from clinical diagnostic criteria and combined with traditional Chinese medicine theory, it is believed that although its core pathogenesis is stagnation of liver qi, yang qi exhaustion is its basic pathogenesis, and deficiency of liver qi leads to the deficiency of liver controlling conveyance and dispersion, and stagnation of liver qi is the manifestation, mental disorders impairing liver qi and deficiency of kidney yang leads to deficiency of liver qi are the root causes. Therefore, when treating depression it not only needs to dispersing stagnated liver qi for relieving qi stagnation and strip qi movement, but also warmly invigorating kidney yang and benefit liver qi, in order to provide a new method to treat depression to improve the situation of treating depression.
