Summary of TCM master DENG Tie-tao's experience in treating vascular dementia with therapy of benefiting qi-expelling phlegm-promoting blood circulation

摘　　要：文章总结国医大师邓铁涛教授运用"益气除痰活血法"治疗血管性痴呆临床经验,邓老认为本病病位在脑,与五脏相关,尤以脾气虚关系密切,其基本病机为脾气亏虚为本,以痰瘀痹阻脑络邪实为标,导致髓减脑消,神机失用。邓老根据本病虚实夹杂病机特点,认为治疗上应以通补兼施,强调益气健脾为主,除痰活血为辅。并主张治疗痰瘀同病时重在化痰,辅以祛瘀,喜用四君子汤合温胆汤临证化裁。文章详细介绍了邓老处方用药及验案。This paper summarizes professor DENG Tie-tao＇s clinical experiences in treating vascular dementia（VD） with therapy of benefiting qi-expelling phlegm-promoting blood circulation. He thinks that the disease location of VD is the brain, which related to the five zang viscera, and closely related to deficiency of spleen qi especially. The basic pathogenesis of this disease are deficiency of spleen qi with phlegm and blood stasis blocking brain collaterals, and then leads to encephala reduction and spiritual dysfunction. Based on the pathogenic characteristics as intermingling deficiency and excess, professor DENG proposes that the treatment of this disease should be given priority to combination of dredging and tonifying, benefiting qi and strengthening spleen, and supplemented with expelling phlegm and promoting blood circulation. He also advocates that the treatment for phlegm and blood stasis should be given priority to remove phlegm and supplemented with removing blood stasis, and with the commonly used prescriptions as combination of Sijunzi Decoction and Wendan Decoction. This paper discusses professor DENG＇s prescriptions and provides proven cases in detail.
