循证护理在大面积软组织损伤VSD引流中的应用

摘　　要：目的探讨循证护理在大面积软组织损伤VSD引流中的应用。方法通过对52例大面积皮肤软组织损伤应用VSD引流术患者的实际情况进行分析,应用计算机网络检索有关文献,查阅相关资料,寻找证据,对资料的可靠性,实用性进行分析、评价,并将所获得的证据和护理专业技能、临床经验、患者的愿望和需求相结合,制定并实施护理方案。结果 52例患者经过循证后实施有针对性的护理措施,44例患者引流未出现并发症,8例患者出现堵管现象,其中2例堵管处理无效,重新更换VSD引流系统。50例患者经VSD引流术后均见创面肉芽组织生长良好,1例并发厌氧菌感染,1例VSD引流无效后行清创截肢术。结论通过对大面积皮肤软组织损伤应用VSD引流患者的循证护理实践,促进护士自觉寻找具有科学依据的护理实证来指导工作,对VSD可能出现的问题及并发症采取措施进行干预,预防和减少不良反应及并发症的发生,保证了患者的安全和治疗效果。Objective To explore the application of evidence-based nursing for patients with large-area skin soft tissue injury treated with vacuum sealing drainage（VSD）drainage.Methods The application of evidence-based nursing for 52 patients with large-area soft tissue injury treated with VSD drainage was analyzed.Releveant literature was collected online from and otther reference materials and the data were used in combination with nurses＇ professional skills,clinical experience and patients＇ wishes and needs so as to make and apply nursing programs.Results 52 cases received evidence-based nursing measures.44 patients showed no complications after drainage,8cases showed tube blocking（including two cases of tube blocking treated by replacing VSD drainage system）.Granulation tissues grew well on the surface of wound after VSD drainage in 50 cases.One case got anaerobic bacteria infection,and one case underwent debridement amputation after the VSD drainage was invalid.Conclusion Evidence-based nursing practice can be conducted for patients with large-area skin soft tissue injury treated with VSD drainage.Patients＇ safety and curative effects can be secured by nurses＇ search for scientific evidence to be used for the guidance of nursing practice.
