某市二级以上公立医院感染性疾病科建设及感染管理现状调查

摘　　要：目的调查广西南宁市二级以上公立医疗机构感染性疾病科建设及其运行情况,为持续改进及卫生行政部门决策提供循证科学依据。方法采用现场调查及发放问卷的方法,调查南宁市卫生和计划生育委员会所属二级以上公立医疗机构34所。内容包括感染性疾病科的设置、布局流程、人员值守、培训、个人防护、手卫生、医疗废物处理等。结果 34所医院中有24所设置感染性疾病科,其中设立传染病预检分诊点的医院为100%,布局流程及通风基本合理占17.6%,具备消毒隔离条件的占97.1%,预检分诊点配备专职人员24 h值守的仅占5.9%,手卫生设施配备合格率为79.4%,传染病防控知识培训、个人防护用品配备、专用车辆转运传染病病人、医疗废物交集中处置单位、每天对工作环境进行清洁消毒等均为100%。结论南宁市大部分医疗机构,尤其是二级以上公立医疗机构能按要求设置感染性疾病科并开展日常工作,但由于各种原因在设置及感染管理方面仍存在一些问题,必须加大投入并落实相关要求,持续改进管理质量。Objective To investigate construction and operation of infectious disease department of Grade-Ⅱ and/ or higher level public hospitals in Nanning of Guangxi,and to provide evidence-based scientific basis for continuous improvement and decision making of health administrative department.Methods 34 Grade-Ⅱ and/ or higher level public hospitals affilia-ted to Nanning Municipal Health and Family Planning Commission were investigated by site surveys and questionnaires.The investigation included establishment of infectious disease department,layout process,personnel on duty,training,personal pro-tection,hand hygiene,medical waste treatment,etc.Results 24 out of 34 hospitals set up infectious diseases department, wherein all of them established infection diseases pre-triage point（100%）,17.6% had reasonable layout and ventilation flow, 97. 1% had disinfection and isolation conditions,and only 5.9% had full-time staff who were 24 h on duty in infection disea-ses pre-triage point.Qualification rate of hand hygiene facilities was 79.4%.In addition,all the hospitals with infectious disea-ses department had infectious disease prevention and control knowledge training,personal protective equipment,dedicated ve-hicle which transport patients with infectious diseases,intersection of medical waste disposal unit,and every day working envi-ronment cleaning and disinfection（all 100%）.Conclusion Most of the hospitals especially Grade-Ⅱ and/ or higher level public hospitals set up infection diseases department and carry on daily work according to requirements.However,there are still some problems in establishment and infection management because of various reasons.Thus,investment should be in-creased,relevant requirements should be implemented,and quality of management should be continuously improved.
