关节镜下内固定术治疗老年胫骨骨折疗效观察

摘　　要：目的探讨关节镜下内固定术在老年胫骨骨折患者的治疗效果,尤其是对膝关节功能恢复的影响。方法选择SchatzberⅠ~Ⅳ型老年胫骨骨折患者为研究对象,随机分为对照组与观察组,对照组患者采用单纯骨折切开内固定复位术,观察组患者采用关节镜下内固定复位术,比较两组患者随访期膝关节功能评分及恢复优良率差异。结果术后3个月随访时对照组与观察者患者膝关节功能评分无差异：15.21vs16.05（P=0.121）,术后6月、12月随访时观察组患者膝关节功能评分显著高于对照组患者：23.82vs21.67（P=0.021）,26.17vs23.05（P=0.006）;观察组患者恢复优良率为82.9%,显著高于对照组的59.5%,P=0.029。结论对老年胫骨骨折患者采用关节镜下内固定术有助于术后回复,疗效较为肯定。Objectives To investigate the therapeutic effect of arthroscopic internal fixation in the treatment of tibial plateau fracture. Methods The elderly patients with tibial fractures were randomly divided into the control group and the observation group and the patients were treated by open reduction and internal fixation with single fracture and internal fixation. Comparison of two groups of patients with follow-up period knee function score and recovery rate difference. Results At the 3 month follow-up, there was no difference between the control group and the knee function score of the patients in the observation group （P = 0. 121 ） , but the observation group was significantly higher than that in the control group at the 6 month and 12 month（ P 〈 0. 05 ）. The recovery of the observation group was better than that of the control group（ P = 0. 029 ）. Conclusions In the treatment of tibial plateau fracture pa- tients with arthroscopie internal fixation is helpful to postoperative recovery.
