香薰推拿减痛配合改良水针镇痛在产程中的应用

摘　　要：目的:比较香薰推拿减痛配合改良水针镇痛在产程中的应用疗效。方法:选择正常足月产妇1000例,单胎头位,骨盆外测量正常,无妊娠合并症和并发症,有阴道分娩条件,随机分为观察组和对照组各500例。两组均采用全程陪伴方法和自由体位,观察组分娩过程中采用香薰推拿减痛配合改良水针镇痛,对照组予常规阴道分娩方式不进行任何干预,两组在第一产程始、末测定血清β-内啡肽浓度,并对两组妊娠结局进行观察比较分析(两组的产痛、产程时间、产后出血量、剖宫产率、新生儿评分)。结果:与对照组比较,观察组第一产程末血清β-内啡肽浓度明显升高,产痛明显减轻,潜伏期时间、活跃期时间及总产程时间明显加快,产后2h出血量明显少于对照组,差异有统计学意义(P<0.01)。新生儿APgar评分比较差异无统计学意义(P>0.05)。结论:香薰推拿减痛配合改良水针镇痛,有效缓解产妇疼痛,降低剖宫产率,操作简单,易于推广。Objective： Comparing the application effect of combining aromatherapy massage to reduce the pain combined with improved water acupuncture analgesia in the course of production. Methods： Choosing 1000 cases of normal full-term birth,single fetal head position,pelvic external measurement was normal,no pregnancy complications and complications,and vaginal delivery conditions,were randomly divided into observation group and controlled group,500 cases in each group. Two groups both used accompanied method and free position,the observation group was given with aromatherapy massage to reduce the pain was improved in the process of childbirth water analgesia,the controlled group was given with conventional way of vaginal delivery without any intervention,at the end of the first labor,determination of serum beta-endorphin concentration,and the comparative analysis of two groups of pregnancy outcomes were observed of the two groups（ two groups of labor pain,labor time,postpartum blood loss,cesarean section rate,neonatal score）. Results： Compared with the controlled group,serum beta-endorphin concentration at the end of the first stages of production of the observation group increased significantly,the labor pain significantly reduce,the incubation period of time,active time and speeding up the total labor time,2h postpartum haemorrhage amount was less than the controlled group obviously,the difference was statistically significant（ P〈0. 01）. Neonatal APgar score comparison difference had no statistical significance（ P〉0. 05）. Conclusion： Using aromatherapy massage to reduce the pain combined with improved water acupuncture analgesia,can effectively relieve maternal pain,reduce cesarean section rate,simple operation,easy to promote.
