微创经皮锁定加压钢板内固定术治疗胫骨中下段闭合性骨折的疗效分析

摘　　要：目的评析微创经皮锁定加压钢板内固定术治疗胫骨中下段闭合性骨折的临床效果。方法现从该院2013年6月—2015年2月诊疗的胫骨中下段闭合性骨折患者中方便筛选出116例,随机分为对照组与治疗组各58例,分别采取切开复位内固定术、微创经皮锁定加压钢板内固定术进行救治,统计比较两组患者手术治疗相关指标的差异,并展开为期6个月的随访,记录患者骨折愈合时间,并评估其膝、踝关节功能恢复情况。结果治疗组的手术切口长度、手术耗时、术中失血量及术后消肿时间与对照组相比均明显更少(P<0.05);治疗组的膝踝关节功能恢复优良率为87.9%,与对照组的68.9%相比明显更高(P<0.05),骨折愈合时间则明显短于对照组(P<0.05)。结论针对胫骨中下段闭合性骨折给予微创经皮锁定加压钢板内固定术治疗可明显减少对正常骨组织与骨折端血液循环的医源性破坏,有利于骨折的愈合,术后膝踝关节功能恢复效果好。Objective To evaluate and analyze the clinical effect of minially invasive percutaneous locking compression plate osteosynthesis in treatment of middle and lower closed tibial fracture. Methods 116 cases of patients with middle and lower closed tibial fracture diagnosed and treated in our hospital from June 2013 to February 2015 were selected and randomly divided into two groups with 58 cases in each, the control group were treated with open reduction and internal fixation, the treatment group were treated with minially invasive percutaneous locking compression plate osteosynthesis, the differences in indexes related to operative treatment of the two groups were counted and compared, the fracture healing time of patients was recorded and the recovery of knee and ankle function was evaluated after 6 months of follow-up. Results The operative cut length, operative time-consuming, intraoperative blood loss and postoperative apocatastasis time in the treatment group were obviously less than those in the control group（P〈0.05）, the excellent and good rate of recovery of knee and ankle function in the treatment group was obviously higher than that in the control group（87.9% vs 68.9%）,（P〈0.05）, and the fracture healing time in the treatment group was obviously shorter than that in the control group（ P 〈0.05）. Conclusion Minially invasive percutaneous locking compression plate osteosynthesis in treatment of middle and lower closed tibial fracture can obviously reduce the latrogenic destruction of normal bone tissue and blood circulation at the end of the fracture,and it contributes to the fracture healing and has a good recovery effect on postoperative knee and ankle function.
