胸腰椎爆裂骨折保守治疗与后路手术治疗的中长期疗效比较

摘　　要：目的探讨胸腰椎爆裂骨折的保守治疗与后路手术的中长期疗效。方法整群选取该院2014年1月—2015年1月收治的108例胸腰椎爆裂骨折的患者作为观察对象,随机分为观察组对照组各54例,观察组患者采用后路复位固定融合术治疗,对照组患者采用保守治疗法。观察两组患者的治疗效果和并发症的发生情况。结果观察组患者的骨折椎局部后凸角度和椎管狭窄率在早期的改善情况较好,组间差异有统计学意义(P<0.05);观察组的并发症总发生率为12.9%,对照组的总发生率16.7%,差异无统计学意义(χ2=0.2935;P>0.05)。结论采用后路手术治疗胸腰椎爆裂骨折的疗效在治疗前期明显高于传统保守治疗疗效,治疗后期两种治疗方法均有较好的治疗效果,两种方法来治疗胸腰椎爆裂骨折均有较高的可行性。Objective To observe the mid-and-long term curative effect of conservative treatment and posterior approach treatment for thoracolumbar burst fractures. Methods 108 cases of patients with thoracolumbar burst fractures admitted and treated in our hospital from January 2014 to January 2015 were selected as the observation object and randomly divided into two groups with 54 cases in each, the observation group were treated with posterior reduction fixation and fusion, the control group were treated with conservative treatment, the treatment effects and occurrence of complications were observed between the two groups. Results The improvement of vertebral fracture local kyphosis angle and spinal canal stenosis rate in early was good in the observation group, and the difference between groups was statistically significant, P〈0.05, the total incidence rate of complications was 12.9% in the observation group and 16.7% in the control group, and the difference was statistically significant（χ2=0.2935;P〉0.05）. Conclusion In the early stage of treatment, the curative effect of posterior approach in treatment of thoracolumbar burst fractures is obviously higher than that of traditional conservative treatment, and both treatment methods have a better effect in the late stage of treatment and have higher feasibility in treatment of thoracolumbar burst fractures.
