摘 要:目的通过经颅多普勒超声(transcranial Doppler ultrasound,TCD)与磁共振血管成像(magnetic resonance angiograghy,MRA)的对比分析,探讨TCD诊断颅内动脉狭窄的特异性和敏感性。方法边续收集2013年6月—2015年6月于该院神经内科住院的缺血性脑血管病患者,先行TCD检查,1周内行MRA检查,以MRA为诊断标准,计算TCD诊断各血管狭窄的特异性和敏感性。结果共收集符合诊断的缺血性脑血管病患者925例,TCD检出狭窄动脉499条,其中大脑中动脉(MCA)216条。大脑前动脉(ACA)92条,大脑后动脉(PCA)102条,颈内动脉虹吸段(CS)56条,椎动脉(VA)63条,基底动脉(BA)72条。MRA检出狭窄动脉446条,其中MCA 184条。ACA 39条,PCA 85条,CS 49条,VA 38条,BA 51条。TCD诊断MCA、ACA PCA、CS、VA、BA各组血管狭窄的特异性和敏感性分别为92.75%、89.32%,79.53%、75.21%,82.64%、80.27%,97.32%、95.37%,96.27%、91.58%,94.20%、70.35%。结论TCD诊断颅内动脉狭窄具有较高的特异性和敏感性,可作为缺血性脑血管病患者常规初筛检查方法。Objective To discuss the specificity and sensitivity of TCD in diagnosis of intracranial arterial stenosis by comparatively analyzing transcranial Doppler ultrasound and magnetic resonance angiograghy. Methods The patients with ischemic cerebrovascular disease hospitalized in the department of neurology from June 2013 to June 2015 were collected,and were given TCD examination firstly and MRA examination in 1 week, the specificity and sensitivity of various vascular stenoses diagnosed by TCD were calculated according to MRA diagnosis. Results 925 cases of patients with ischemic cerebrovascular disease meeting the diagnosis were collected, 499 stenotic arteries were detected by TCD, including 216 middle cerebral arteries, 92 anterior cerebral arteries, 102 posterior cerebral arteries, 56 siphon carotid arteries, 63 vertebral arteries and 72 basilar arteries; 466 stenotic arteries were detected by MRA, including 184 middle cerebral arteries, 39 anterior cerebral arteries, 85 posterior cerebral arteries, 49 siphon carotid arteries, 38 vertebral arteries and 51 basilar arteries, the specificity and sensitivity of TCD in diagnosis of middle cerebral arteries, anterior cerebral arteries, posterior cerebral arteries, Siphon carotid arteries, vertebral arteries and basilar arteries were respectively 92.75 %, 89.32 %, 79.53 %, 75.21 %,82.64 %, 80.27 %, 97.32 %, 95.37 %, 96.27 %, 91.58 %, 94.20 % and 70.35 %. Conclusion The specificity and sensitivity of TCD in diagnosis of intracranial arterial stenosis are higher, which can be used as the routine preliminary screening method.
