米索前列醇在足月妊娠引产中的效果探讨

摘　　要：目的探讨米索前列醇在足月妊娠引产中的效果。方法方便选取2015年7—12月笔者援助的非洲桑给巴尔奔巴地区Abdalla Mzee医院住院足月待产妇100例,随机分为米索组50例和缩宫素组50例,研究组给予米索前列醇引产,对照组给予催产素引产,比较两组在促宫颈成熟、引产效果及妊娠结局方面的结果。结果 (1)研究组促宫颈成熟达92.0%,对照组64.0%,差异有统计学意义(P<0.01);研究组引产成功率96.0%,对照组引产成功率74.0%,作用明显优于对照组,差异有统计学意义(P<0.01)。(2)研究组与对照组用药至临产时间分别为(7.3±2.6)h、(15.4±4.2)h,总产程分别为(16.4±2.3)h、(20.3±4.5)h,两组比较差异均有统计学意义(P<0.01)。(3)研究组产后2 h出血量、新生儿窒息发生率、新生儿出生体重与对照组比较,差异无统计学意义(P>0.05)。结论米索前列醇在足月妊娠引产中效果确切,具有用药方便,安全有效,价格低廉的优势,值得临床推广应用。Objective To study the effect of misoprostol in term pregnancy induction. Methods 100 cases of full-term expectant mothers hospitalized in the Abdalla Mzee hospital of Zanzibar area in Africa assisted by the writer from July 2015 to December 2015 were selected and randomly divided into two groups with 50 cases in each, the research group were given misoprostol for induced labour, the control group were given oxytocin for induced labour, and the maturing cervix, induced labour effect and pregnancy outcome between the two groups were compared. Results（1） The difference in the maturing cervix rate between the research group and the control group had statistical significance,（92.0% vs 64.0%）（P〈0.01）, the induced labour success rate in the research group was obviously better than that in the control group,（96.0% vs 74.0%）, and the difference had statistical significance（P〈0.01）.（2）The differences in the time from medication to labor and total stage of labor between the research group and the control group had statistical significance by comparison, [（7.3±2.6） h,（16.4±2.3） h vs（15.4±4.2） h,（20.3±4.5） h]（P〈0.01）.（3）The differences in the volume of bleeding during 2 hours after labor, incidence rate of neonatal asphyxia and neonatal birth weight between the research group and the control group had no statistical significance by comparison（P 0.05）. Conclusion The effect of misoprostol in full-term pregnancy induction is definite with convenient medication and cheap price, and it is safe and effective, which is worth clinical promotion and application.
