分析小切口开放式保留肾单位手术治疗肾肿瘤的临床疗效

摘　　要：目的分析小切口开放式保留肾单位（NSS）手术治疗肾肿瘤（中山评分≥8分）的临床疗效。方法选取2010年4月—2015年6月采用小切口NSS治疗62例肾肿瘤（中山评分≥8分）患者,总结分析其临床资料、随访术后肿瘤复发和转移情况。结果 62例患者均顺利完成小切口NSS治疗。手术时间86-160 min,平均（115.4±24.2）min。术中热缺血时间25-40 min,平均（29.6±4.9）min。术中出血量25-450 m L,平均（73.8±65.5）m L。术后2例手术区域局部积液,未发生其他手术相关并发症。术后3月复查血肌酐值和e GFR值为（68.5±20.1）umol/L和（83.5±21.8）ml/（min·1.73 m2）,显著低于术前（P〈0.05）。随访6~24月未见肿瘤复发和转移。结论小切口开放式NSS治疗中山评分≥8分肾肿瘤临床疗效确切,具有较高临床应用价值。Objective To analyze the clinical curative effect of mini-incision open type nephron sparing surgery treatment for renal tumor（Zhongshan score ≥8 marks）. Methods 62 cases of patients with renal tumor（Zhongshan score ≥8 marks）treated with mini-incision NSS from April 2010 to June 2015 were selected, and the clinical data were summarized and analyzed, and the postoperative tumor relapse and metastasis were followed up. Results 62 cases of patients successfully finished the mini-incision NSS treatment, the operation time was 86-160 min, the average operative time was（115.4 ±24.2）min, the intraoperative warm ischemia time was 25-40 min, and the average intraoperative warm ischemia time was（29.6±4.9） min, the intraoperative blood loss was 25-450 ml, and the average intraoperative blood loss was（73.8 ±65.5） ml, after operation, local effusion in operative region occurred to 2 cases, and there was no complications in relation to operation, and the postoperative 3-month reexamination showed that the serum creatinine value and e GFR value were respectively（68.5 ±20.1） umol/L and（83.5±21.8） ml/（min﹒1.73m2）, which were obviously lower than those before operation（P 〈0.05）, and the tumor relapse and metastasis did not occur after 6-24 months follow-up. Conclusion The clinical curative effect of miniincision open type nephron sparing surgery treatment for renal tumor（Zhongshan score ≥8 marks） is definite, which is of higher clinical application value.
