皮瓣移植联合负压封闭引流术治疗长期慢性溃疡创面的应用研究

摘　　要：目的探讨皮瓣移植联合负压封闭引流（VSD）治疗长期慢性溃疡创面的临床效果。方法整群选取2014年6月—2015年11月该院收治的96例老年长期慢性溃疡患者,按照信封法将其随机划分为对照组（n=48）和观察组（n=48）,对照组按照传统方法治疗,即先清创、后换药;观察组采用皮瓣移植联合应用VSD技术治疗,根据两组患者创面愈合情况及新生肉芽情况对两组患者进行疗效评定并进行比较。结果观察组患者在采用VSD治疗5-7 d后,均形成鲜红致密颗粒状的肉芽,在采用皮瓣移植术后,治疗总有效率为95.83%,对照组总有效率为62.50%,两组差异有统计学意义。结论皮瓣移植联合负压封闭引流术治疗长期慢性溃疡,效果显著,方便易行,值得临床推广应用。Objective To observe the clinical effect of skin flap transplantation combined with vaccum sealing drainage in treatment of long-term and chronic skin ulcer wound. Methods 96 cases of elderly patients with long-term and chronic skin ulcer wound admitted and treated in our hospital from June 2014 to November 2015 were selected and randomly divided into two groups with 48 cases in each according to the envelope method, the control group were treated with traditional method, namely at first debridement, then dressing, the observation group were treated with skin flap transplantation combined with vaccum sealing drainage, and the curative effects of the two groups were evaluated and compared according to wound healing situations and neonatal granulation situations. Results The bright-red and dense granular granulations were formed after 5-7d of VSD treatment in the observation group, after skin flap transplantation, the difference in the total treatment effective rate between the observation group and the control group had statistical significance（95.83% vs 62.50%）.Conclusion The effect of skin flap transplantation combined with vaccum sealing drainage in treatment of long-term and chronic ulcer is obvious, and it is easy and convenient and is worth clinical promotion and application.
