巨大子宫肌瘤应用腹腔镜辅助阴式全子宫切除术治疗的效果分析

摘　　要：目的分析巨大子宫肌瘤应用腹腔镜辅助阴式全子宫切除术治疗的临床效果。方法随机选取2014年2月—2016年2月该院收治的70例巨大子宫肌瘤患者临床资料,根据数字随机法,把70例患者分成对照组与观察组,各35例。对照组应用开腹全子宫切除术,观察组应用腹腔镜辅助阴式全子宫切除术,对比两组患者手术时间、术中出血量及术后排气时间、下床时间、住院时间等情况。结果观察组手术时间(119.5±15.6)min、术中出血量(117.2±66.3)m L、术后排气时间(22.6±9.8)h、下床时间(1.65±0.88)d、住院时间(5.25±1.00)d,对照组手术时间(82.5±8.6)min、术中出血量(146.5±73.5)m L、术后排气时间(46.5±11.2)h、下床时间(2.95±0.83)d、住院时间(7.62±1.08)d,观察组手术时间多于对照组,观察组术中出血量、术后排气时间、下床时间、住院时间等情况明显比对照组短,两组对比差异有统计学意义(P<0.05)。结论巨大子宫肌瘤患者应用腹腔镜辅助阴式子宫切除术效果更为理想,可以明显缩短手术时间,减少术中出血量,利于患者术后恢复,建议推广应用。Objective To analyze large fibroids laparoscopic assisted vaginal hysterectomy for clinical effect. Methods Randomly chosen from February 2014 to February 2016 in our hospital 70 cases of clinical data of patients with huge uterine fibroids, according to figures randomly to 70 patients divided into control group and observation group, 35 patients in each.The control group abdominal hysterectomy, observation group laparoscopically assisted vaginal hysterectomy, comparing two groups of patients surgical time, blood loss and postoperative exhaust time, ambulation time, hospital stay and so on. Results Observation operative time（119.5 ± 15.6） min, blood loss（117.2 ± 66.3） m L, postoperative discharge time（22.6 ± 9.8）h, bed time（1.65 ± 0.88） d, length of hospital stay（5.25 ± 1.00） d, the control group operative time（82.5 ± 8.6） min, blood loss（146.5 ± 73.5） m L, postoperative discharge time（46.5 ± 11.2） h, bed time（2.95 ± 0.83） d, length of stay（ 7.62 ± 1.08）d,Observe the operation time than the control group, the observation group blood loss, postoperative exhaust time, ambulation time, length of stay, etc. significantly shorter than the control group, a significant difference between two groups（P〈0.05）.Conclusion The patients with uterine fibroids huge laparoscopy assisted vaginal hysterectomy better results can significantly shorten the operation time and reduce blood loss, which will help patients recover, the proposed application.
