灵性照顾在社区临终关怀病房中应用的研究

摘　　要：目的探讨社区卫生服务中心在临终病人中实施全面灵性照顾。方法选取2014—2015年社区卫生服务中心符合入选标准的肿瘤终末期病人59例,组建灵性照顾团队,以FICA工具、询问灵性病史,明确生命意义,信仰、希望、内疚、宽恕,疾病、死亡接纳,爱及社会连接等6个维度灵性问题。根据具体需求给予相应的灵性照顾。干预前后以McGill生活质量问卷以及心理痛苦温度计为评价工具。结果进行灵性照顾后各组病人幸福指数均有显著提高,接受期病人尤其显著。干预后癌症患者生活质量、身体症状、心理情况等方面均比干预前差(P<0.05);干预前患者McGill各分量以及总分均大于干预后的McGill评分(P<0.05);干预前癌症患者心理痛苦程度较干预后程度高(P<0.05)。结论实行规范化灵性照顾在社区病房是可行的,有很大的可操作性,能显著改善患者的生活质量和幸福状态。Objective To investigate the community health service center in hospice patients in the implementation of a comprehensive spiritual care. Methods 2015 community health service center conforms to the selected standard tumor pa-tients with end-stage 59 cases, formation of spiritual care team, to FICA tools, consult the spiritual history, clear meaning of life, faith, hope, guilt and forgiveness, sickness, and death, and accept, love and agency will connected to the six dimensions of spiritual matters. According to the specific needs to give the corresponding spiritual care. Before and after the interven-tion to Mc Gill quality of life questionnaire and psychological distress thermometer as evaluation instrument. Results The spiritual care of patients in each group after the happiness index increased significantly, especially for patients to accept. Dry prognosis of cancer patients quality of life, physical symptoms and psychological condition and so on are poorer than before intervention （P〈0.05）. Intervention patients before McGill each component and the total score were higher than dry prognosis of McGill score （P〈 0.05）. Intervention of cancer patients psychological pain degree than the dry high degree of prognosis（P〈0.05）. Conclusion The standardized spiritual care in the community hospital is feasible and has great maneu-verability, can significantly improve the patient＆#39;s quality of life and well-being.
