集束化护理策略对维持性血透患者高磷血症的影响与分析

摘　　要：目的探讨与分析集束化护理策略对维持性血透患者高磷血症的应用效果。方法采用便利取样的方法,选取2014年7—12月在怀化市某三甲医院血透中心的维持性血透患者作为研究对象。共纳入65例,采用集束化护理措施干预6个月后,对血透患者的血磷水平及血浆白蛋白浓度的变化进行调查和分析。结果①维持性血透患者的血磷浓度,干预前为(2.55±0.52)mmol/L,干预6个月后为(2.18±0.63)mmol/L(t=5.594,P<0.05);②血透患者的血浆白蛋白浓度显示,干预前(41.81±6.34)g/L,干预6个月后为(42.16±3.58)g/L,(t=-0.461,P<0.05);③患者的性别和年龄与血磷和血浆白蛋白的变化,显示无显著相关性(P>0.05)。结论结果提示集束化护理干预措施,可有效改善维持性血透患者的高磷水平和提升患者的血浆白蛋白值。Objective To investigate and analyze the application effect of nursing strategy on maintenance hemodialysis pa-tients with hyperphosphatemia. Methods By convenience sampling, from July 2014 to December 2014 in Huaihua City, a hospital hemodialysis center in maintenance hemodialysis patients as the research object. A total of 65 cases, the nursing intervention after 6 months, through the change of serum phosphate levels and plasma albumin concentrations in blood were investigated and analyzed. Results ① serum phosphorus concentration in patients with maintenance hemodialysis, before the intervention （2.55 ± 0.52） mmol/L, 6 months after the intervention was （2.18 ± 0.63） mmol/L （t= 5.594, P〈 0.05）; ②plasma albumin levels in hemodialysis patients, before intervention （41.81 ± 6.34） g/L, 6 months after the intervention was （42.16 ＋ 3.58） g/L, （t=-0.461, P〈 0.05）; ③ changes in patients with gender and age and serum phosphorus and serum al-bumin, showed no significant correlation （P〉 0.05）. Conclusion The results suggest that the nursing intervention measures can effectively improve the patients with maintenance hemodialysis in patients with high levels of phosphorus and enhance the value of albumin.
