摘 要:目的探讨HLA-DRB1位点等位基因多态性与朝鲜族、汉族女性不明原因复发性自然流产(unexplained recurrent spontaneous abortions,URSA)的相关性。方法选取2013年10月至2015年10月发生URSA的朝鲜族女性82例及汉族女性78例纳入研究组,同时选取于本院就诊的正常孕早期朝鲜族女性80例及汉族女性88例纳入对照组。采用聚合酶链反应-序列特异性扩增技术(PCR-SSP)对研究对象进行HLA-DRB1基因分型,分析HLA-DRB1等位基因分布频率在朝鲜族、汉族女性中的分布差异及其与URSA的相关性。结果 HLA-DRB1位点各等位基因HLA-DRB1*01、03、04、05、06、07、08、09、10、11、12、13、14、15、16在研究组朝鲜族和汉族女性中的分布频率均无显著差异(P>0.05),且在对照组朝鲜族和汉族女性中分布频率亦无显著差异(P>0.05);研究组HLA-DRB1*03、04、06、09、11的分布频率分别为8.12%、10.00%、6.88%、11.88%、7.50%,均显著高于对照组(2.38%、3.57%、1.19%、3.57%、1.79%)(P<0.05)(OR分别为1.94、2.37、4.94、2.76、3.59);HLA-DRB1*05、12在研究组中的分布频率分别为6.25%、8.75%,均显著低于对照组(14.29%、18.45%)(P<0.05)(OR分别为0.43、0.48)。结论 HLA-DRB1*01、03、04、05、06、07、08、09、10、11、12、13、14、15、16等位基因在朝鲜族和汉族女性中的分布频率无显著差异;HLA-DRB1*03、04、06、09、11可能是URSA发病的易感基因,HLA-DRB1*05、12可能是URSA发病的保护性基因。Objective To investigate the correlation between polymorphism of HLA-DRB1 locus and unexplained recurrent spontaneous abortions (URSA) in Korean and Han women. Method From October 2013 to October 2015, 82 Korean nationality cases of URSA patients and 78 Han nationality cases of URSA were selected as research group, and 80 cases of healthy Korean women and 88 cases of healthy Han nationality from pregnancy fertility clinic were selected as control group. The PCR-SSP was used as the genotyping method to study HLA-DRB1 gene. Analyzed the distribution of HLA-DRB1 allele frequency in different ethnic groups and its correlation with URSA. Result The HLA-DRB1 alleles distribution in Han and Korean women: In research group, there was no significant difference of the frequency distribution of HLA- DRB1*01, 03, 04, 05, 06, 07, 08, 09, 10, 11, 12, 13, 14, 15, 16 between Korean and Han women (P 〉 0.05);and in control group, the frequency distribution between Korean and Han women had also no significant difference (P 〉 0.05). The HLA-DRB1 allele frequency distribution in research group and control group: in research group, the gene frequencies of HLA-DRB1*03, 04, 06, 09, 11 were 8.12%, 10.00%, 6.88%, 11.88%, 7.50%, respectively, which were significantly higher than control group (2.38%, 3.57%, 1.19%, 3.57%, 1.79%) (P 〈 0.05) (OR = 1.94, 2.37, 4.94, 2.76, 3.59, respectively); the gene frequencies of HLA-DRB1*05, 12 in&amp;nbsp;research group were 6.25% and 8.75%, which were significantly lower than control group (14.29%, 18.45%) (P 〈 0.05) (OR = 0.43, 0.48, respectively). Conclusion The distribution frequencies of HLA-DRB1*01, 03, 04, 05, 06, 07, 08, 09, 10, 11, 12, 13, 14, 15, 16 alleles between Korean women and Han women are not significantly different; HLA-DRB1*03, 04, 06, 09, 11 may be the susceptible gene of URSA and HLA-DRB1*05, 12 may be the protective gene of URSA.
