摘  要:目的探讨经阴道三维超声对先天性子宫畸形患者的临床诊断价值。方法 2008年7月至2015年7月,对西安市中心医院52例疑似先天性子宫畸形患者分别行经阴道二维及三维超声检查,对比分析两种诊断方法的准确率。结果经阴道三维超声对先天性子宫畸形患者诊断准确率为100%(52/52),其中双角子宫18例,纵隔子宫12例,弓形子宫10例,单角子宫7例,残角子宫5例;经阴道二维超声对先天性子宫畸形的诊断准确率为86.54%(45/52)。经阴道三维超声下可观察到双角子宫的子宫内膜回声均匀,宫颈管合于子宫中下段至宫颈口处;纵隔子宫分隔未到达宫颈口的患者子宫内膜呈"Y"型,而分隔达子宫颈口的呈"V"形;弓形子宫中央部内膜呈弧形凹陷;单角子宫内膜呈管状或半月状;残角子宫可观察到宫旁肌性团块,可有或无子宫内膜,内膜与发育侧子宫腔可相连或不相连。结论经阴道三维超声能清晰、直观地显示子宫宫腔形态结构及宫底外观轮廓,对于子宫畸形的诊断准确率优于二维超声,是一种准确、易于操作的诊断方法。Objective To investigate the clinical diagnostic value of three-dimensional transvaginal ultrasound in patients with congenital uterine malformations. Method From July 2008 to July 2015, took two-dimensional and three-dimensional transvaginal ultrasound examinations on 52 patients with suspected congenital uterine malformation and compared the diagnostic accuracy of the two methods. Result The diagnostic accuracy of three-dimensional transvaginal ultrasound in patients with congenital uterine malformations was 100% (52/52), in which were 18 cases of double uterine horn, 12 cases of mediastinal uterus, 10 cases of arcuate uterus, 7 cases of unicornuate uterus, and 5 cases of rudimentary horn uterine. The diagnostic accuracy of two-dimensional ultrasound for congenital uterine malformations was 86.54% (45/52). Under three-dimensional ultrasound: homogeneous endometrial echo and cervix tube closing at the middle or lower part of uterine to the cervix can be observed in bicornuate uterus; "Y" shape endometrium can be observed in incomplete septate uterus while "V" shape in complete septate uterus; "arc sag" shape endometrium can be observed in the middle of arcuate uterus; tubulous or semilunar shape endometrium can be observed in unicornuate uterus; solid mass near uterus and presence or deficiency of endometrium can be observed in rudimentary horn of uterus with connection or disconnection of endometrium to the developmental uterine cavity. Conclusion The three-dimensional transvaginal ultrasound can clearly visualize the morphology appearance of uterus and uterine fundus and have a higher diagnostic accuracy in the diagnosis of uterine malformations accuracy than two-dimensional ultrasound. It is a simple, accurate and easy diagnostic method.
