全髋关节置换术治疗大骨节病致髋关节骨关节炎:中期随访结果

摘　　要：目的探讨全髋关节置换术治疗大骨节病致髋关节骨关节炎的方法,分析其中期临床随访和影像学结果。方法北京积水潭医院矫形骨科自2002年10月年至2013年1月间对大骨节病致髋关节终末期骨关节炎的8例患者（13髋）行全髋关节置换术,全部患者均采用生物固定型假体进行关节重建,并行回顾分析。临床结果采用Harris髋关节评分进行评价,影像学结果依据术后系列X线影像进行分析。结果平均随访时间77.5个月（25.0~156.0个月）。Harris髋关节评分从术前平均45.6分（36.0~54.0分）提高至术后最后1次随访的91.3分（86.0~98.0分）,差异有统计学意义（P〈0.01）。术后患者未发生假体松动、脱位、深部感染以及假体周围骨折等并发症。结论全髋关节置换术治疗大骨节病致髋关节骨关节炎的中期临床和影像学随访结果令人满意,具有较低的并发症发生率和较高的患者满意度。Objective To explore the medium-term clinical and radiographic results of total hip arthroplasty（ THA） for the treatment of osteoarthritis after Kaschin-Beck disease. Methods A retrospective study was performed on 8 cases of patients（ 13 hips） from October 2002 to January 2013. Cementless cups and stems were inserted in all cases. Clinical results were evaluated by Harris hip score and radiographic results were analyzed by postoperative serial X-ray films. Results The average duration of follow-up was77. 5 months（ range,25-156 months）. The average Harris Hip score increased from preoperative 45. 6（ range,36-54） to 91. 3 points（ range,86-98） at the latest follow-up. No case of aseptic loosening,dislocation,periprosthetic infection or fracture was seen. Conclusion THA is a reliable option for reconstruction of hip osteoarthritis after Kaschin-Beck disease. The complication rate is low and level of patients satisfaction is high.
