摘 要:目的:探究慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)稳定期患者在治疗中慢性阻塞性肺疾病评估测试评分(CAT)、英国医学研究委员会呼吸困难表评分(mMRC)与BODE指数的相关性,并分析采用辛伐他汀干预治疗后的影响效果。方法:选择2013年2月~2014年12月在某院接受治疗的COPD稳定期病患176例,以数字法随机分为观察组和对照组各88例。其中观察组采用常规治疗方法,对照组采用辛伐他汀干预治疗。首先对观察组治疗前后CAT、mMRC与BODE指数的相关性作出评估,然后对比两组之间的相关水平差异。结果:观察组中治疗前后CAT与FEV1%、6MWD存在明显的负相关,与mMRC、BODE指数存在明显的负相关。而mMRC评分与FEV1%存在显著的负相关,与BODE指数存在明显的正相关。两组治疗结果中CAT和BODE指数都出现明显的下降,但治疗后仅有对照组的CAT低于观察组。mMRC评分中两组在分别治疗后处于0、1级的病患比例都出现了显著的增加,并且对照组的比例高于观察组,差异均有统计学意义(均P〈0.05)。结论:COPD稳定期病患在治疗后通过评估发现CAT、mMRC与BODE指数存在明显的相关性,而应用辛伐他汀治疗后CAT、mMRC水平改善明显,可以在临床上适当推广应用。Objective:To explore correlation between BODE,mMRC and CAT in COPD stable phase and the effect of simvastatin on the intervention of Simvastatin.Methods:176patients with stable COPD disease treated in our hospital from February 2013 to December 2014 were randomly divided into observation group and control group with 88 cases in each group.The observation group was treated by routine treatment,and the control group was treated with simvastatin.First,the correlation between mMRC,CAT and BODE index were evaluated before and after treatment,and then the compare the correlation difference between the two groups.Results:In the observation group,there were significant negative correlation between CAT and FEV1%,6MWD,and negative correlation with the BODE and mMRC index.However,there was a negative correlation between mMRC score and FEV1%,which was positively related to the BODE index.The CAT and BODE index of the two groups were decreased obviously,but the CAT of the control group was lower than that in the observation group.The proportion of patients at 0and grade 1after treatment were significantly increased in the mMRC score,and the proportion of the control group was higher than that in the observation group,respectively,and the control group were significantly higher than the observation group.The difference was statistically significant(P<0.05).Conclusion:After treatment,it was found that there was close correlation between BODE,mMRC and CAT in COPD stable phase and after treating with simvastatin,the CAT、mMRC levels are significantly improved,which can be applied and promoted in clinic.
