胫后动脉与足背动脉置管在有创血压监测的应用效果分析

摘　　要：目的胫后动脉与足背动脉置管在新生儿有创血压监测中的应用效果差异。方法选取行有创血压监测130例新生儿作为研究对象,在取得其直系监护人同意后采用随机数字表法将受试患儿分成A和B两组,各65例。B组采用足背动脉置管有创血压方案,A组采用胫后动脉置管有创血压监测方案。比对两组患儿置管留置时间及疼痛评分情况,记录其一次性置管成功率,穿刺点渗血率,血管并发症发生率差异。结果两组患儿在动脉置管时新生儿疼痛评估量表(NIPS)评分对比上均无统计学意义(P>0.05):A组置管时间明显长于B组,差异具有统计学意义(P<0.05):A组一次性置管成功率及血管并发症发生率均明显低于B组,差异具有统计学意义(P<0.05)。结论足背动脉置管及胫后动脉置管均为新生儿行有创血压监测的可行性置管方法,足背动脉一次性置管成功率较高和导管留置时间较短,胫后动脉则穿刺点渗血率较低。Objective To apply the effect difference of the posterior tibial artery and the dorsal artery of the foot in the neonatal invasive blood pressure monitoring. Methods A blood pressure monitoring of 130 cases of newborns as the research object,in the case of their immediate guardians agreed to use a random number table method,the children were divided into A and B two groups,each of 65 cases. In the B group,the blood pressure was set in the dorsal arterial of the foot,and the blood pressure monitoring program was used in the A group. We compared the two groups of patients in indwelling time and pain score,the success rate of the disposable tube,puncture point bleeding rate,vascular complication rate difference. Results In the two groups,the score of neonatal pain assessment scale（ NIPS） was not statistically significant（ P 0. 05）. The B group was significantly longer than that in A group,the difference was statistically significant（ P 0. 05）. The success rate and the incidence of vascular complications in the A group were significantly lower than that in the B group,the difference was statistically significant（ P 0. 05）. Conclusions Dorsalis pedis artery catheterization and tibial artery catheter tube for neonatal invasive blood pressure monitoring the feasibility of catheter method,foot dorsal artery of disposable intubation success rate is high and the catheter indwelling time shorter,posterior tibial artery puncture oozing of blood rate is low.
