高血压合并糖尿病前期炎症因子TNF-α和IL-6水平的研究

摘　　要：目的探讨高血压合并糖尿病前期患者肿瘤坏死因子-α(TNF—α)和白介素-6(IL-6)水平的变化。方法133例入选者分为对照组(39例)、高血压组(34例)、高血压合并糖尿病前期组(60例)。常规检测空腹血糖(FBG)、餐后2h血糖、血脂、糖化血红蛋白(HbA_(1c))、肝肾功能、超敏C反应蛋白,ELISA法检测TNF—α及IL-6水平。结果血浆TNF—α、IL-6水平合并糖尿病前期组分别为(114.68±15.39)pg/ml和(24.58±2.89)pg/ml,高血压组分别为(97.30±13.78)pg/ml、(24.02±2.54)pg/ml,均较对照组[(51.90±7.83)pg/ml、(12.67±2.22)pg/ml]升高(均P<0.01)。TNF—α水平合并糖尿病前期组较高血压组升高(P<0.01)。相关性分析显示,TNF—o与收缩压(SBP)、舒张压(DBP)、HbA_(1c)的相关系数分别是0.769、0.673、0.622(均P<0.01),IL-6与SBP、DBP、HbA_(1c)的相关系数分别是0.798、0.695、0.525(均P<0.01)。多元回归分析表明,血压、FBG及HbA_(1c)是TNF—α和IL-6的独立危险因素(均P<0.05)。结论高血压及糖尿病前期均会导致血浆TNF—α和IL-6水平的增加,高血压合并糖尿病前期患者更明显。Objective To investigate the changes of tumor necrosis factor alpha （TNF- α） and interleukin- 6 （IL- 6） in hypertensive patients with prediabetes. Methods 133 cases were enrol ed and divided into control group（39 cases）, hypertension group（34 cases） and hypertension with prediabetes group（60 cases）. Fasting blood glucose, postprandial glucose, lipid level, HbA1c, renal and hepatic function, and hypersensitive CRP were examined routinely. Serum TNF- a and IL- 6 were measured by ELISA. Results The levels of TNF- a and IL- 6 in hypertension with prediabetes group [ （114.68 ±15.39）pg/ml, 24.58 ±2.89] pg/ml, respectively] and hypertension group [（97.30 ±13.78）pg/ml, （24.02 ±2.54） pg/ml] were significantly higher than in control group [（51.90±7.83）pg/ml, （12.67±2.22）pg/ml ]（al P〈0.01）. TNF- a was significantly higher in hypertension with prediabetes group than in hypertenison group （P〈0.01）. The correlation analysis showed that the correlation coefficients between TNF- a and SBP, DBP, HbA1c were 0.769, 0.673, 0.622, respectively （P〈0.01）, and those between IL- 6 and SBP, DBP, HbA1c were 0.798, 0.695, 0.525, respectively （al P〈0.01）. The multiple regression analysis showed that blood pressure, FBG, HbA1c were the independent risk factors of TNF- a and IL- 6 （al P〈0.01）. Conclusion Serum TNF- a and IL- 6 increase in both Hypertension and prediabetes, and more significantly in hypertension with prediabetes.
