3种磺脲类药用于2型糖尿病初始治疗的疗效与短期经济学评价

摘　　要：目的:探讨3种磺脲类药用于2型糖尿病初始治疗的短期经济学效果。方法:选取2012年8月—2015年8月牡丹江医学院红旗医院收治的2型糖尿病患者180例作为研究对象,以随机数字表法分为格列美脲组、格列喹酮组和格列吡嗪组,每组各60例。格列美脲组患者给予格列美脲4 mg,1日1次;格列喹酮组患者给予格列喹酮15 mg,1日3次;格列吡嗪组患者给予格列吡嗪15 mg,1日1次。12周为1个疗程,治疗过程中检测血糖及糖化血红蛋白(Hb A1c)。结果:格列美脲组患者的总有效率显著高于格列吡嗪组,格列美脲组患者餐后2 h血糖下降百分率、Hb A1c下降率均显著高于格列喹酮组、格列吡嗪组,差异均有统计学意义(P<0.05);将评价指标设定为餐后2 h血糖下降百分率,格列美脲组、格列喹酮组比格列吡嗪组分别增加了8.44、17.54元;将评价指标设定为Hb A1c下降率,格列美脲组、格列喹酮组比格列吡嗪组分别增加了36.49、53.96元;餐后2 h血糖下降百分率、Hb A1c下降率均不敏感于药品价格;3组患者的不良反应发生率之间的差异均无统计学意义(P>0.05)。结论:格列美脲较格列喹酮与格列吡嗪初治2型糖尿病短期经济学效果好。OBJECTIVE：To probe into the short-term economic effects of 3 kinds of sulfonylurea drugs in the initial treatment of type II diabetes .METHODS： 180 patients with type II diabetes admitted into Hongqi Hospital of Mudanjiang Medical University from Aug .2012 to Aug .2015 were selected to be divided into glimepiride group , gliquidone group and glipizide group via the random number table , with 60 cases in each .Patients in glimepiride group were treated with glimepiride 4 mg, once a day;the gliquidone group were given gliquidone 15 mg, 3 times a day;and the glipizide group received glenn pyrazine 15 mg, once a day; 12 weeks for a treatment course .And the blood glucose , HbA1 c were determined during the treatment process .RESULTS：The total effective rate of glimepiride group was was significantly higher than that of glipizide group;the decent percentage of blood glucose after 2 h and HbA1 c of glimepiride group were significantly higher than those of gliquidone group and glipizide group , with statistically significant difference （ P 〈0.05 ） .To set the indicators as decent percentage of 2 h postprandial blood glucose , compared with glipizide group , glimepiride group, gliquidone group had increased by 8.44 yuan, 17.54 yuan.And to set the indicators as decent percentage of HbA 1 c, compared with glipizide group , glimepiride group , gliquidone group had increased by 36.49 yuan, 53.96 yuan.The decent percentage of 2 h postprandial blood glucose and HbA 1 c were insensitive to the price of drugs; the differences of incidence of adverse drug reactions among three groups were not significant （P〉0.05）.CONCLUSIONS：The short-term economics effects of glimepiride is better than gliquidone and glipizide in the initial treatment of type 2 diabetes.
