医院-家庭-社区健康教育管理对老年糖尿病患者胰岛素治疗自我管理的影响

摘　　要：目的探讨对空巢老年糖尿病患者进行医院-家庭-社区健康教育管理的方法和效果。方法选取2014年1月—2015年1月在内分泌科住院的空巢老年糖尿病患者40例。在患者住院期间,组织患者参加病区的糖尿病教育课堂,根据患者的具体情况进行个体化教育指导,并对患者照顾者进行相同的健康教育;出院后,护士对患者每月进行1次随访;将患者的信息转往所属社区卫生服务中心,由社区护理人员对患者进行小组教育和上门随访。对患者的胰岛素治疗相关知识、胰岛素注射技能掌握情况和血糖控制情况进行评价。结果患者对胰岛素相关知识(F=5.625,P=0.003)和胰岛素注射(F=8.843,P<0.001)的每季度考核分数逐步升高;干预后患者空腹血糖(t=10.002,P<0.001)、餐后2h血糖(t=8.677,P<0.001)和糖化血红蛋白(t=22.998,P<0.001)数值均低于干预前。结论对空巢老年糖尿病患者进行医院-家庭-社区健康教育管理有助于患者掌握胰岛素治疗知识和胰岛素注射技能,从而有效控制血糖水平,提高其生活质量。Objective To evaluate the effect of hospital-family-community health education on self management of insulin therapy among empty nest elderly patients with diabetes. Methods Forty empty nest elderly patients with diabetes admitted to hospital during January 2014 to January 2015 were recruited in this study. During the period of hospitalization,the patients and relatives participated in diabetes education class and received individualized education guidance. After discharge,nurses provided follow-up service for the patients once a month. The patients were transferred to a community health service center,and were followed up by community nurses. Patients＇ insulin therapy knowledge,insulin injection skill and the control of blood glucose were evaluated. Results After intervention,patients＇ insulin therapy knowledge（F=5.625,P=0.003） and insulin injection skill（F=8.843,P〈0.001） was improved significantly. The fasting blood glucose（t=10.002,P〈0.001）,postprandial 2h blood glucose（t=8.677,P〈0.001） and glycosylated hemoglobin（t =22.998,P 〈0.001） decreased. Conclusion The hospital-family-community diabetes health education mode can improve insulin therapy knowledge and insulin injection skill,and the control of blood glucose among empty nest elderly patients with diabetes.
