螺内酯联合雷公藤对合并亚临床甲减的早期2型糖尿病肾病患者尿微量白蛋白的影响

摘　　要：目的:分析螺内酯联合雷公藤对合并亚临床甲状腺功能减退(甲减)的早期2型糖尿病肾病患者尿微量蛋白的影响。方法:以80例早期2型糖尿病肾病合并亚临床甲减患者为研究对象,将患者随机平均分为观察组和对照组,对照组单纯使用雷公藤治疗,观察组患者在此基础上使用小剂量螺内酯治疗,对比两组患者的尿微量白蛋白水平。结果:治疗后两组患者的24h UAlb、TSH水平显著降低,治疗前后比较差异有统计学意义(P<0.05);治疗前后Scr水平差异不明显。治疗后,观察组患者的24h UAlb水平显著低于对照组,24h UAlb水平差异有统计学意义(P<0.05)。治疗期间,观察组出现低血糖反应1例,对照组出现低血糖反应2例。结论:螺内酯联合雷公藤可显著降低合并亚临床甲减的早期2型糖尿病肾病患者的尿蛋白水平,且安全可靠。Objective： Analyze spironolactone combined tripterygium wilfordii of merger of subclinical hypothyroidismearly type 2 diabetic nephropathy patients with urinary micro protein.Methods：80 cases of early in patients with type 2 diabeticnephropathy merger subclinical hypothyroidism as the research object,the patients were randomly divided into observation groupand control group,control group simply using tripterygium wilfordii treatment,observation group of patients on the basis of usingsmall doses of spironolactone treatment,compared two groups of patients＇ urine trace albumin levels.Results：The two groups aftertreatment in patients with 24 hualb,TSH level decreased significantly,compare the difference was statistically significant beforeand after the treatment（P〈0.05）;No obvious difference Scr levels before and after the treatment.After treatment,the observationgroup of patients with 24 hualb were significantly lower than the control group,24 hualb level difference was statisticallysignificant （P〈0.05）.During the treatment,observation group appear hypoglycemia in 1 case,control group appear hypoglycemiain 2 cases.Conclusion：Spironolactone combined TWHF can significantly reduce the merger of subclinical hypothyroidism earlyurine protein levels in patients with type 2 diabetic nephropathy,and safe and reliable.
