不同备皮方法对头皮肿物切除术的影响

摘　　要：目的:本研究采取不同的备皮方式,通过对头皮肿物切除术后手术切口的愈合情况进行研究,探寻备皮方式,为神经外科头皮肿物切除术术前头部皮肤准备提供依据。方法:随机选取42例海军总医院全军神经外科研究所拟行头皮肿物切除术的患者,将42例患者随机分为术前常规全脑毛发剃除组和肿物局部毛发剃除组。对术后手术切口的愈合情况进行分析,探讨两种备皮方法对手术切口愈合的影响的差异。结果:术前两组患者年龄、性别未见明显差异(P>0.05),全脑毛发剃除组手术时间与局部毛发剃除组手术时间两组间手术时间未见明显统计学差异(P>0.05)。术前头皮损伤程度两者存在统计学差异(P<0.05),术中出血量两组间未见明显差异(P>0.05)。术后全脑剃除组Ⅰ/甲级愈合19例,局部剃除组Ⅰ/甲级愈合17例,两组术后手术切口愈合未见明显差异(P>0.05)。结论:头皮肿物局部备皮对皮肤损伤小,与全脑备皮对手术切口愈合的影响无明显统计学差异,局部备皮头皮损伤小,患者心理负担小。Objective： Preoperativeskin preparation is recognized as an important method to reduce the rates of infectionafter scalp tumor surgery.Preoperative hair shave with the whole head is widely applied.Different methods of preoperative skinpreparation are studied to investigate a new way of skin preparation according to the healing of incision following scalp tumorsurgery.Methods：Forty-two patients preparing for the scalp tumor resection were randomly selected in Naval General hospital.Twenty-three cases were randomly divided into the whole hairdressing group,and nineteen cases into the group with part ofhairdressing.Effect of preoperative skin preparation between two groups was investigated through analysis of healing of incisionafter the surgery.Results：There was no significant difference between the two groups in age,sex,operation time and amount ofbleeding during operation （P〉0.05）.Degree of scalp injury in the group with part of hairdressing was more slightly than thecontrol group （P〈0.05）. 17 incisions primarily healed were found in the part of hairdressing group comparing with 19 in thewhole hairdressing group with no significant difference.Conclusion：Part of hairdressing can be applied in scalp tumor surgerywith a smallersmaller skin damage and less psychological burden.
