摘 要:目的:通过对人体血浆中环境雌激素含量的检测,从临床流行病学角度探讨其与子宫肌瘤的关联性。方法:以张家港及南京地区汉族子宫肌瘤患者为病例组,非子宫肌瘤者为对照组,进行一般人口学调查,采集血液标本,应用高效液相串联质谱法(HPLC-MS)对血浆中双酚A(BPA)、辛基酚(OP)、壬基酚(NP)等酚类环境雌激素进行定量检测,分析比较两组间差异。结果:病例组214例,对照126例。两组间酚类环境雌激素暴露水平、准确检出人数及浓度分布范围等存在不同。Logistic分析发现,NP暴露水平(OR为1.059,95%CI为1.002-1.120)与子宫肌瘤发病率的关联性具有统计学意义(P〈0.05)。结论:NP暴露可能是子宫肌瘤发病的危险因素,酚类环境雌激素的人体暴露水平与子宫肌瘤的发生存在一定的相关性。Objective: To investigate whether environmental estrogens( EEs) of human plasma are associated with potential reduced risk of uterine fibroids. Methods: We collected plasma samples and investigated general demographic data from a case control study conducted in the No. 1 People's Hospital of Zhangjiagang and Zhongda Hospital of Southeast University. High performance liquid chromatography tandem mass spectrometry( HPLC-MS)was used for quantitative detection of bisphenol A( BPA),octylphenol( OP),nonylphenol( NP) and other phenolic compounds in the blood. The differences between the case group and the control group were analyzed and compared. Results: We collected 214 cases of uterine myoma patients as case group,and 126 cases of non-uterine myoma subjects as control group. The results showed that there were some differences in the exposure to environmental estrogens,the exactly detected number of people and the range of concentration between the two groups. Logistic regression analysis showed that NP exposure( OR 1. 059,95% CI 1. 002-1. 120) was associated with the onset of uterine fibroids. Conclusion: NP exposure may be the risk factor of uterine fibroids. The exposure to environmental estrogen has certain correlation with the occurrence of uterine fibroids.
