摘 要:目的:临床上比较半导体激光和强脉冲光脱毛的临床疗效、舒适度及安全性;在动物实验中观察黑毛陆川猪激光后的组织学改变来探讨激光和强光的脱毛机制。方法:患者左侧腋毛和唇毛为半导体激光治疗组,右侧腋毛和唇毛为强脉冲光治疗组,观察3次治疗时的毛发直径、密度、疗效指数、疼痛指数、患者主观满意度、脱毛设备选择的倾向性及不良反应;分别以半导体激光和强脉冲光对小型陆川猪后颈不同区域进行单次治疗,以光镜和电镜观察脱毛后即刻及3周时的组织学改变。结果:无论使用哪种脱毛设备,随着治疗次数增加,患者毛发密度皆变稀、直径变细,疗效指数亦随治疗次数增加而提高,其差异无统计学意义(P>0.05);前3次脱毛的疼痛程度皆为强脉冲光设备较半导体激光设备疼痛,其差异有统计学意义。动物实验证实,两种不同激光设备皆对毛囊细胞及其胞器有不同程度的损伤,而皮脂腺、汗腺等皮肤附属器未见改变。结论:研究结果显示,两种设备对脱毛皆有良好的效果且疗效指数无明显差异,但半导体激光的舒适度更好,色沉更少,更多患者倾向使用半导体激光设备完成后续的脱毛疗程;半导体激光和强脉冲光皆可通过损伤毛囊细胞,抑制毛发生长,实现脱毛疗效。Objectives: To make a comparison between diode laser and intense pulsed light(IPL)hair removal in clinical efficacy, comfort, and safety. To observe the changes of histology after hair removal and thus investigate the hair removal mechanism of diode laser and intense pulsed light(IPL)used in an animal experiment: a small black Luchuang pig. Method: one treatment group was treated with diode laser to remove the left armpit and lip hair; the other with IPL to remove the right armpit and lip hair. In three times of treatment, closely observed were hair diameter and density, index of therapeutic effect, pain index, subjective satisfaction of patients, selection of hair removal device intended, and adverse reactions were closely obserred. Respectively, light microscope and electron microscope were used to observe the histological changes immediately and three weeks after hair removal. Results: No matter what kind of hair removal device was used, with the increase in the number of treatment, the patients' hair was getting thin both in density and diameter; efficacy index also on the rise with the increasing number of treatment, and the difference was not statistically significant (P〉0. 05). As to the degree of pain, IPL device proved sharper than diode laser in the previous three times of hair removal; the difference was statistically significant. Animal experiments also confirmed that two different laser devices inflicted varying degrees of damage on hair follicle cells and organelles, but not on other skin appendages such as sebaceous glands and sweat glands. Conclusion: the research results show that two devices have good effects on hair removal and the therapeutic indexes have no significant difference. But diode laser is better in comfort and less in hyperpigmentation, thus attracting more patients to use diode laser to complete the subsequent removal treatments.
