应用超声心电图诊断感染性心内膜炎的临床价值

摘　　要：目的探讨超声心动图应用于感染性心内膜炎患者的诊断价值,为临床诊治提供参考。方法选取2012年1月-2015年1月于医院行手术治疗45例感染性心内膜炎住院患者为研究对象,对患者进行超声心动图检查与血培养,并将超声检查结果与手术病理检查结果进行比较。结果 21例患者血培养结果为阳性,其中以葡萄球菌最高感染率占47.62%,其次为链球菌感染占38.10%;经超声心动图检查,显示有32例患者存在赘生物,75.76%的患者心脏赘生物直径为2-15mm,87.88%的患者心脏赘生物数量≤3个;超声心动图对瓣膜脱垂、瓣周漏、腱索断裂的检出率均为100.0%,脓肿、瓣膜穿孔检出率分别为75.0%、57.14%。结论感染性心内膜炎患者应用超声心动图检查可了解心脏赘生物所在部位、直径大小与数量,为临床诊断提供可靠依据。OBJECTIVE To explore the value of echocardiography in diagnosis of patients with infectious endocarditis so as to provide guidance for clinical diagnosis and treatment.METHODS A total of 45 hospitalized patients with infectious endocarditis who underwent surgeries from Jan 2012 to Jan 2015 were recruited as the study objects,then the echocardiographic examinations and blood culture were conducted for the patients,and the results of the echocardiographic examinations were compared with the results of pathological examinations.RESULTS Totally 21 patients showed positive for blood culture,of whom 47.62% had Staphylococcus infection,and 38.10% had Streptococcus infection.The echocardiographic examination showed that 32 patients had neoplasms,of whom75.76% had cardiac neoplasms with the diameter varying from 2to 15 mm,and 87.88% had no more than 3cardiac neoplasms.The detection rates of valve prolapse,perivalvular leakage,and tendon rupture were 100.0%;the detection rates of abscess and valve perforation were 75.0% and 57.14%,respectively.CONCLUSION The echocardiography may facilitate the understanding of the locations,diameters,and quantities of the cardiac neoplasms of the patients with infectious endocarditis,which provides reliable basis for clinical diagnosis.
