摘  要:目的探讨含氢乳酸钠林格液在同系小鼠异位心脏移植过程中对冷缺血再灌注损伤的作用。方法制备含氢浓度为4mg/L饱和乳酸钠林格液。18只小鼠随机分为3组:(1)普通组:以普通乳酸钠林格液作为心脏保存液进行心脏移植;(2)氢气组:以含氢乳酸钠林格液作为心脏保存液进行心脏移植;(3)空白组:仅麻醉小鼠后切开颈部皮肤再缝合,不进行心脏移植。比较氢气组与普通组心脏移植后复跳时间。在移植心脏再灌注24h比较各组的组织形态学、细胞凋亡、心肌酶、炎症及氧化应激水平。结果氢气组与普通组心脏移植后的复跳时间无显著差异(P=0.32)。再灌注24h,与普通组比较,氢气组的凋亡细胞显著减少,差异有统计学意义(P〈0.01)。与普通组比较,氢气组的间质水肿、心肌结构及炎症细胞浸润有显著改善。与普通组比较,氢气组的血清天冬氨酸转氨酶、乳酸脱氢酶、肌酸激酶、白细胞介素1β、白细胞介素6、肿瘤坏死因子α、单核细胞趋化蛋白1和移植物组织丙二醛水平显著降低,差异均有统计学意义(P〈0.01)。氢气组的透明质酸合成酶-1和高迁移率族蛋白B-1水平也明显低于普通组,差异有统计学意义(P〈0.05)。结论含氢乳酸钠林格液能减轻小鼠心脏移植冷缺血再灌注损伤,其机制可能是抗氧化应激、抗凋亡及抗炎作用。Objective To investigate the effect of hydrogen-rich sodium lactate Ringer's solution on cold ischemia-reperfusion injury of heart transplantation in syngeneie mice. Method Sodium lactate Ringer's solution saturated with hydrogen at a concentration of 4 ppm was prepared. Eighteen mice were randomly divided into three groups: (1) common group: heart transplantation was performed and sodium lactate ringers solution served as heart preservation solution; (2) hydrogen group: heart transplantation was performed and hydrogen-rich sodium lactate Ringer's solution served as heart preservation solution; (3) blank group: mice were anesthetized, and the skin of the neck was incised and sutured without heart transplantation. The resuscitation time in hydrogen group and common group was compared. Histopathology, cell apoptosis, myocardial enzymes, inflammatory factors, and oxidative stress markers of each group were compared at 24 h after reperfusion. Result There was no significant difference in the resuscitation time between hydrogen group and common group (P = 0. 32). At 24 h after reperfusion, the apoptotic cells in hydrogen group were significantly fewer than in common group (P〈0. 01). The interstitial edema, myocardial structure and inflammatory cell infiltration were significantly relieved in hydrogen group as compared with common group. The serum aspartate aminotransferase, lactate dehydrogenase, creatine kinase, interleukin-lβ, interleukin-6, tumor necrosis factor-a, monocyte chemoattractant protein-1 levels and tissue malondialdehyde levelswere significantly reduced in hydrogen group as compared with common group (P〈 0. 01 ). The endogenous ligands hyaluronic acid synthase-1 and high mobility group protein B1 were also significantly reduced in hydrogen group as compared with common group (P〈0. 05).
