结肠造口灌洗及口服微生态制剂对肠造口患者肠道菌群及生活质量的影响

摘　　要：目的观察结肠造口灌洗及口服微生态制剂对结肠造口患者术后肠道菌群调节及生存质量的影响。方法选取2012年1月-2015年1月江苏大学附属医院普外科行腹会阴联合直肠癌根治术病例58例,随机分为观察组29例和对照组29例,对照组采用自然排便法,观察组采用结肠造口灌洗法加口服微生态制剂。在术前、术后首次排便、术后2周、术后1个月行肠道菌群培养计数,同时行术后2周、术后1个月采用生活质量核心量表进行问卷调查并进行比较。结果在术后两周、术后1个月时,观察组肠道中性菌(肠球菌、肠杆菌)和有益菌(双歧杆菌、乳酸杆菌)较对照组均不同程度升高且差异有统计学意义(P<0.05);观察组术后生活质量明显优于对照组(P<0.05)。结论腹会阴联合直肠癌根治术后,造口灌洗及口服微生态制剂可以改善结肠造口患者术后肠道微生态环境,提高患者的生活质量。Objective To observe the effect of colostomy irrigation and oral administration of probiotics on the intestinal flora and the life quality of patients underwent colostomy. Methods Between January 2012 and January 2015, a total of 58 patients underwent abdominal perineal joint colorectal cancer radical operation （Miles operation） in our hospitals were selected and randomly divided into observation group （29 cases） and control group （29 cases）. The control group used natural defecation, and the observation group used colostomy irrigation and oral administration of probiotics. Intestinal flora of patiens was fosted and counted at preoperative period, the first time of postoperative defecation, two weeks and one month after operation. At the time of two weeks and one months after the operation, the core quality of life scale questionnaires were measured and compared in both groups. Results At the time of two weeks and one month after operation, intestinal bacteria of the observation group was significantly greater than the control group （P 〈 0.05）. The quality of life in the observation group was significantly better than the control group （P 〈 0.05）. Conclusions After Miles operation, colostomy irrigation and oral administration of probiotics can improve intestinal micro-ecological environment and the quality of life of the colostomy patients.
