癫痫合并抑郁障碍的中医证候分析

摘　　要：目的探讨癫痫合并抑郁障碍患者的中医证候特征。方法收集癫痫合并抑郁障碍患者156例,进行中医四诊检查,按症状分为轻度、中度、重度和严重4级,按照中医辨证标准记分。对采集的症状进行频数统计,对变量进行聚类分析。结果癫痫合并抑郁障碍患者中医证候分为以下3类:肝郁痰凝型(38.6%):发作时喉中痰鸣、心烦、抑郁、口干、胁肋胀痛、善太息、恶心、口黏、身体困重、舌红、弦脉;精髓亏虚型(36.9%):头痛、烦躁、健忘、肢冷、动作迟钝、腰酸、形体痿软、尿频、虚脉;心脾两虚型(24.5%):失眠、神疲、乏力、易惊、畏寒、胆怯、舌淡、细脉。结论癫痫合并抑郁障碍患者中医证候较复杂,但以肝郁痰凝、精髓亏虚、心脾两虚证型多见。Objective To discuss TCM syndromes features in patients with epilepsy with depressive disorder. Methods 156 patients suffering from epilepsy with depressive disorder were classified, into with four diagnostic methods, they were classified into mild, moderate, severe and extreme severe conditions and scored by standard TCM differentiation. Frequency statistics were made on the collected symptoms and cluster analysis were used to analyze the variables. Results These patients could be divided into 3 types： liver qi stagnation and phlegm stasis syndrome（38. 6% ） ： phlegm in the throat during the seizure, irritability, depression, dry mouth, distending pain in the hypochondriac region, sighing, nausea, sticky mouth, heaviness of the body, red tongue, string pulse; marrow deficiency syndrome（36. 9% ）： headache, irritability, poor memory, cold limbs, retarded movement, soreness of lumber, atrophy body, frequent urination, feeble pulse; heart and spleen deficiency syndrome（24. 5% ）： insomnia, fatigue, weakness, being easily frightened, fear of cold, timidity, pale tongue and thready pulse. Conclusion The symptoms of epilepsy with depressive disorder were very complex with three commonly-seen syndromes, including liver qi stagnation and phlegm stasis syndrome, marrow deficiency syndrome, and heart and spleen deficiency syndrome.
