摘  要:目的 进一步了解腓动脉穿支皮瓣的分布规律,为皮瓣的合理应用提供显微外科解剖学基础.方法 对10例成人新鲜小腿标本进行红色乳胶灌注,观测腓动脉穿支数目、分布、蒂长、外径、走行等情况,并进行统计学分析,归纳其规律性.结果 10例标本共计47支穿支,平均4.7支,主要分布在腓骨头下10.0~30.0 cm范围内;腓动脉穿支近段穿支以比目鱼肌肌穿支为主,中段以坶长屈肌肌穿支为主,而远段以肌间隙穿支为主.结论 腓骨头下10.0~30.0 cm节段腓动脉穿支较为恒定,穿支蒂较长,外径较粗,适合设计切取腓动脉穿支皮瓣游离移植.腓动脉有四处穿支出现基本恒定,血管管径在1.0 mm左右,适宜血管吻合,血管长度约5.0 cm,有足够的长度作为皮瓣血管蒂,适合皮瓣设计.Objective To clearly define the blood vessels distribution in the peroneal artery perforators flapand provide anatomic basis for the application of peroneal artery perforator flap. Methods Ten flesh lower leg specimens of adults were injected with red latex and were dissected. The number, distribution, and the courses of the peroneal artery perforators were explored, also, the pedicle length and external diameter of the artery used for the flap were measured and analyzed. Results 47 perforators were identified (4.7 perforators for each leg averagely). The perforators in the proximally lateral leg originated from different arteries. Peroneal artery perforator gathered in the region under the fibular head about 10 to 30 cm. The perforators in the proximal lateral leg were mainly soleus perforators, and most of septocutaneous perforators and the flexor hallucis longus muscle perforators gathered in the lateral mid-distal region. Conclusion There are 4 regular perfemtors with stable course, enough size and length.peroneal artery in lateral calf, blood vessel diameter is 1 mm or so, appropriate vascular anastomosis, vascular static length is about 5 cm. Perforators originate from the region under the fibular head about 10 to 30cm, is ideal nutrient vessel for supplying flee peroneal perforator flap.
